Objective: through the comparison based on the theory of "emphasis on both sinews and bones" percutaneous minimally invasive internal fixation with steel plate technology and interlocking intramedullary nails in the treatment of clinical curative effect of the middle and lower segment tibia fractures, analyze the advantages and disadvantages of two kinds of operation, can be better for the clinical treatment of the middle and lower segment tibia fractures. Methods: met inclusion criteria and exclusion criteria of the first affiliated hospital of shandong university of TCM orthopedics in October 2015 to April 2017,treated and follow-up of 60 patients with fracture of tibial middle section is divided into two groups: group of percutaneous minimally invasive plate fixation with interlocking intramedullary nailing group,30 cases in each group, two groups of surgery are to follow the theory of "emphasis on both sinews and bones".Through analysis and study of above two types of internal fixation in the treatment of lower segment tibia fractures in the ease of operation, operation time, intraoperative bleeding, perspective, flow rate and postoperative clinical fracture healing time, postoperative complications, etc.,to summarize the clinical curative effect of the two kinds of operative methods. Results: patients in both groups received 6~12 months of follow-up,and finally all bone healing.Based on the theory of "emphasis on both sinews and bones" of the two kinds of minimally invasive techniques in the degree of difficulty on interlocking intramedullary nail technology is more complex, more clinical experience and skills in the process of operation, and the operation time, intraoperative fluoroscopy times, clinical healing time were more than minimally invasive percutaneous plate osteosynthesis, two compared with statistical significance(P<0.05),but the two in the intraoperative and postoperative bleeding, drainage on the difference was not statistically significant(P>0.05).In addition,Although the totalincidence of postoperative complications in the percutaneous minimally invasive plate fixation group was lower than that in the intramedullary nail group, the rate of excellent postoperative efficacy evaluation was higher than that in the intramedullary nail group, the two groups of data were tested by ?2,P > 0.05,with no statistical difference. Conclusion: based on the theory of "emphasis on both sinews and bones" percutaneous minimally invasive plate internal fixation with interlocking intramedullary nails technology is treatment of fractures of the middle and lower tibia a better surgical method, but the former is easy to operate, short operation time, less intraoperative fluoroscopy, less postoperative complications and clinical fracture healing time is short, the more positive clinical therapeutic effect. |
