Objective: To explore the clinical characteristics of late preterm infants(LPI)through by retrospective analysis and comparison of premature infants admitted to neonatal ward of our hospital from January 2017 to December 2019,so as to provide basis for clinical diagnosis and treatment and follow-up management,and then improve the quality of life of LPI.Methor: The preterm infants hospitalized in the Department of Neonatology of the First Affiliated Hospital of Guangxi Medical University from January 2017 to December 2019 were collected.842 premature infants of32~36+6 weeks gestational age at birth were selected from the department of obstetrics and postnatal neonatology.On the basis of the gestational age,there were 708 cases of LPI at 34-36+6 weeks of gestational age and 134 cases of moderate premature infants(MPI)at 32-33+6 weeks of gestational age.The hospitalization data and follow-up data of the two groups were collected,and the SPSS25.0 software was used for retrospective analysis and comparison.The general conditions,the incidence of complications during hospitalization,the general conditions of hospitalization and the intelligent development after discharge were compared between the two groups.Results: 1.During the study period,LPI accounted for 74.1 % of all hospitalized preterm infants.2.The incidence of amniotic fluid abnormalities,intrauterine distress,maternal adverse pregnancy history and gestational hypertension in LPI were lower than MPI(P<0.05).3.Although LPI has a high incidence of diseases,its incidence of respiratory distress syndrome,infectious pneumonia,feeding intolerance,periventricular-intraventricular hemorrhage,neonatal purulent meningitis,sepsis,hyperbilirubinemia,neonatal anemia,and retinopathy of prematurity are all lower than MPI(P<0.05).4.The average hospitalization time,blue light therapy,blood transfusion therapy,peripherally inserted central venous catheters(PICC),ventilator assisted respiration and intravenous hypernutrition rate of LPI were lower than MPI(P<0.05).5.At the corrected gestational age was of 1 month and 3 months,there was a certain lag in intelligence development between LPI and full-term infant standard(DQ<85),which had better motor,language ability,fine movement,adaptability and social ability than MPI(P<0.05).At corrected gestational age of 6 months,LPI intelligence development score was close to normal level(DQ≥85),MPI was still behind normal level,and LPI language ability and social ability were better than MPI(P<0.05).At corrected gestational age of 9 months,the scores of LPI and MPI intelligence development reached the normal level(DQ≥85),and the fine movement of LPI was better than MPI(P<0.05).At the corrected gestational age of 12 months,the LPI intelligence development score was higher than the normal level(DQ≥85 points),but lagged behind MPI(P>0.05).6.At the corrected gestational age of 1 month and 3 months,there was a certain degree of intellectual development lag between the EUGR group and the full-term infant standard(DQ<85).The motor,language ability,fine movement,adaptability and social ability of the EUGR group were lower than the non-EUGR group(P<0.05).At the corrected gestational age of 6 months,the intelligence development score of EUGR group was still lower than that of normal group(DQ<85),and the motor,fine movement,language ability and adaptability of EUGR group were lower than the non-EUGR group(P<0.05).At the corrected gestational age of 9 months,the intelligence development score of the EUGR group was close to the normal standard level(DQ≥85),and the motor was still lagging behind the non-EUGR group(P<0.05).At the age of 12 months,the intelligence development scores of the two groups reached the normal level(DQ≥85),but the language ability and adaptability of the EUGR group were still lower than the non-EUGR group(P<0.05).Conclusion: 1.LPI accounted for a large proportion of hospitalized premature infants.2.There was a high incidence of diseases in LPI.3.LPI has a risk of recent intellectual backwardness in early postnatal period,which was easier to catch up with development than MPI.4.The risk of mental retardation increased,when the LPI was combined with EUGR. |
