Research On The Influencing Factors And Moxibustion Intervention Effect Of Comfort Of Patients With Indwelling Catheter

Objective1.Tough Cross-sectional study to understand the main symptoms and incidence of discomfort in patients with indwelling catheter through the questionnaire survey and analyze the main factors that affect the comfort of patients,and explore the intervention methods that can effectively promote the comfort of patients with indwelling catheter,and provide theoretical basis for the development of intervention programs and implementation of interventions.2.To observe the effect of moxibustion on the comfort of patients with urinary tract irritation and pain of urethra mouth,so as to further improve the comfort of patients with indwelling catheter during the period of carrying the catheter and provide scientific basis and reference for formulating the standardized process of moxibustion applied to patients with indwelling catheter.Methods1.Cross-sectional study:To prepare the questionnaire of influencing factors on the comfort of indwelling catheter.Through retrieving up the factors related to the comfort of the urinary tube,the questionnaire was initially prepared.After consulting experts by correspondence,the items that isn’t closely related to the research in the questionnaire were modified and deleted to form the final questionnaire,including the patient version and the nurse version.To understand the main symptoms and influencing factors of discomfort in patients with indwelling catheter through the questionnaire survey.From January 2016 to June 2017,117 patients with indwelling catheter were selected as the subjects of study.In order to control bias,patients with head and neck diseases are selected,and patients with urological diseases,lower abdominal diseases,general internal medicine and surgery,neurology and surgery departments with mental and consciousness influence are excluded.2.Clinical trial: The patients with indwelling catheter were treated with moxibustion.According to the investigation results of the first part of the study,consult the literature,consult the experts,draw up the operation plan and form the operation standard process.From July 2017 to June 2018,61 patients with indwelling catheter in Otolaryngology,Head and Neck Surgery,The First Affiliated Hospital of Air Force Medical University were selected.According to the sequence of patients’ operation,the patients were divided into control group and treatment group.When the patient is awake,the control group was given routine care.The treatment group added moxibustion treatment of three points including Zhongji point,Guanyuan point and Qihai point,on the basis of routine care.Each acupuncture point was moxibustion for 5min,a total of 15 min.After the intervention,to understand the factors influencing the comfort of patients with indwelling catheter,such as urinary tract irritation,pain of urethra orifice,influence of activity,time of first micturition after extubation,whether to induce micturition during micturition and whether to conduct second catheterization by questionnaire.Results1.The incidence rate of uncomfortable symptoms in patients with indwelling catheter were irritation of urinary tract,pain of urethral orifice,bad mood and other discomfort,they were 83.76%,31.62% and 91.45% respectively.Among them,bad mood and other discomfort are the most common.2.The main factors influencing the comfort of patients with indwelling catheter.The components of the injected fluid and the fixed position of the catheter affect the irritation sign of the urinary tract;the depth of re insertion after the catheter is inserted into the urine affects the pain of the urethra;the causes of the catheter affect the mood and other discomfort of the patients with the catheter(P < 0.05).3.Evaluation of the effect of moxibustion intervention on the comfort of patients with indwelling catheter.After the intervention of moxibustion,it was found that the effect of the treatment group on the stimulation sign of urinary tract,the influence of activity,the time of the first micturition after the removal of the catheter,whether the induced micturition was carried out were better than that of the control group.The difference between the two groups was statistically significant(P<0.05);There was no statistically significant between the two groups in the results of urethral orifice pain and secondary catheterization(P > 0.05).Conclusion1.The main factors influencing the comfort of patients with indwelling catheter were the composition of the liquid injected into the indwelling catheter air bag,the depth at which the catheter was inserted after seeing urination,the fixed position of the catheter and the causes of indwelling catheter.2.The application of moxibustion at Zhongji,Guanyuan and Qihai points can improve the comfort of patients during the period of catheter retention,short the time interval of first urination after catheter removal,reduce the number of induced urination,and improve the comfort of patients.
