ObjectiveTo understand the sleep quality of patients with chronic venous insufficiency(CVI),explore the factors affecting sleep quality,In-depth understanding of sleep disorders and coping behaviors in CVI patients, and provide an effective basis for formulating effective interventions to improve patients’ sleep quality.Methods1.The quantitative study adopted the convenience sampling method, and selected CVI patients who were admitted to the Department of Vascular Surgery of the Second Affiliated Hospital of Soochow University from January to December 2019 as the research subjects.330 patients were surveyed by questionnaire and symptom-based management was used. The theoretical self-made lower extremity venous insufficiency questionnaire, clinical severity scale(VCSS),and Pittsburgh Sleep Quality Index(PSQI)collect data. The t-test,and χ2 test were used for single factor analysis, and the two-category Logistic regression analysis was used to obtain the main influencing factors.SPSS20.0 used as the statistical software,P<0.05 is meaningful.2.The qualitative study adopted phenomenological research methods, and useed the largest difference sampling method to extract samples from 330 patients in the quantitative study.Semi-structured interviews were conducted on 11 CVI patients with sleep disorders to gain in-depth understanding of CVI patients with sleep disorders. The experience of sleep disorders and coping behaviors are analyzed using the Colaizzi 7-step analysis method to analyze the interview data and refine the theme.Results1.A total of 330 CVI patients were included in the quantitative study, including 189 males and 141 females, and 145(43.9%)had sleep disorders. The PSQI score was 9.6±2.27.There were 185 people(56.1%)without sleep disturbance, and the PSQI score was 5.84±0.96.The distribution of PSQI scores≥2 points in each dimension is:the main sleep quality dimension score≥2 points,a total of 142 people(43.0%),the sleep disorder dimension score≥2 points,a total of 141 people(42.7%),and the daytime dysfunction dimension score A total of 99 people(33.0%)with≥2 points,a total of 90 people(27.3%)with a score of falling asleep time dimension≥2 points,a total of 77 people(23.3%)with a sleep time dimension score≥2 points,61 people(18.5%)with a sleep efficiency dimension score≥2 points,9 people(2.7%)hypnotics dimension score≥2 points.2.Logistic regression analysis showed that gender, age, nap, elevated limbs, years of onset, pain, frequency of attacks, and VCSS score were related to sleep disorders in CVI patients(P<0.05).3.The qualitative research data extracted the following three themes:bad sleep experience, helplessness with sleep problems, choice of helpless endurance, lack of knowledge, and expectation of timely and effective management.Conclusions1.Poor sleep quality in CVI patients is mainly manifested as:poor subjective sleep quality, night sleep disturbance, and daytime dysfunction.2.The risk factors for sleep disorders in CVI patients are female, advanced age, nap, raising the affected limb, long years of onset, pain, high frequency of attacks, and high severity of venous clinical.3.CVI patients are experiencing bad sleep. On the basis of fully understanding the current sleep quality and risk factors of CVI patients, active and effective intervention measures should be taken targeted, and multi-content and multi-form health guidance should be given to effectively improve sleep quality.
