| Objective:To compare and analyze the clinical curative effect of traditional Chinese medicine comprehensive therapy and operation therapy on Neer II and III type proximal humeral fractures in elderly patients with osteoporotic and to summarize the advantages and disadvantages of the two methods for the treatment of proximal osteoporosis Neer II and III type proximal humeral fractures Provide the basis for the treatment.Methods:70 cases of elderly patients with osteoporotic Neer II and III type proximal humerus fractures admitted to our hospital orthopedic clinic(emergency)and inpatient department from October 2015 to February 2017 were selected and randomly divided them into two groups : 35 cases in the treatment group and 35 cases in the operation group,including 33 males and 37 females,aged from 61 to 92 years with a median age of 72.4 years.The patients were followed up for 9-17 months with an average of(12.50 ± 4.70)months.Causes of injury: 47 cases of walking injury or injury,injuries to 14 cases of hardware,traffic accidents in 7 cases,2 cases of height fall.Neer type of fracture type,including 32 cases of type II fracture,type III fracture in 38 cases.Two groups of patients in gender,age,causes of injury,fracture sites and other aspects of the difference was not statistically significant,P> 0.05,comparable.Traditional Chinese medicine treatment group on the treatment of plastic surgery,small splint external fixation with traditional Chinese medicine three times to give traditional Chinese medicine fumigation and oral treatment of fistula.Surgical treatment group was given conventional open reduction and proximal humeral locking plate fixation,postoperative conventional treatment.Two groups of patients admitted to the hospital were routine anti-osteoporosis drug alendronate dimensional D3 tablets(Familia,Merck East,Import Drug Registration No.H20130840)orally,each 1,once a week.After treatment,both groups were instructed to shoulder functional rehabilitation exercise.Compare the two groups of patients after fracture reduction,fracture healing time,shoulder joint recovery and the incidence of complications.Results:All 70 patients were followed up for 12-17 months with an average of(13.50 ± 4.70)months.The fracture reduction,fracture healing and 3 months and 6 months after treatment in both groups were statistically significant Function score comparison,comprehensive treatment group were better than surgical treatment group,the difference was statistically significant,P <0.05.There was no significant difference in the Neer score,the excellent rate of shoulder joint and the complication rate between the two groups at 12 months after treatment(P> 0.05).Conclusion:Application of traditional Chinese medicine comprehensive therapy and surgical treatment to treat osteoporosis Neer II type,type III proximal humeral fractures of elderly patient all can be achieve a satisfactory effect.Surgical treatment group had advantages in fracture reduction,and traditional Chinese medicine comprehensive treatment group had some advantages in patients with recent recovery of shoulder joint function and fracture healing time.In addition,the comprehensive treatment of traditional Chinese medicine also has trauma,low cost,simple treatment,patient compliance and other advantages,it is worth application on clinical. |
