Objective To observe the expression of AQP1,AQP3 and AQP5 in degenerative lumbar intervertebral disc and its clinical effect.The mechanism of lumbar intervertebral disc degeneration was analyzed.The combination of clinical observation and clinical observation could provide some ideas and references for the treatment of low back pain.Methods The model of lumbar intervertebral disc degeneration was established by lumbar puncture in rabbits.The expression of AQP1,AQP3 and AQP5 in intervertebral disc was detected by RT-PCR and immunohistochemistry.The changes of model rabbits after 6 weeks of administration were observed.The patients with clinical low back pain were collected and treated with physiotherapy,physiotherapy+Qiangjing Zhuanggu Qufeng heji and physiotherapy + Yaotongning respectively for 1 month.The VAS score,JOA score and RSI were observed after treatment.Results The basic experimental part: 1.RT-PCR detection of AQP1 expression:blank control group,sham operation group,model blank group,Yaotongning group,Qiangjinzhuanggu heji group,the m RNA level of each group has significant difference(F=48.483,P=0.000).There was no significant difference between the sham operation group and the blank control group(P=0.230),the model blank group,the Yaotongning group,the Qiangjin Zhuanggu heji group compared with the blank control group(P= 0.000,0.001,0.012,0.009),the model blank group,the Yaotongning group,the Qiangjin Zhuanggu heji group,the differences were significant(P=0.000,0.000,0.001),Yaotongning group,Qiangjin Zhuanggu heji group comparison,the difference was significant(P=0.004,0.006).The expression of AQP3 was detected by RT-PCR.The m RNA expression of AQP3 in blank control group,sham operation group,model blank control group,Yaotongning group and Qiangjin Zhuanggu heji group were all significantly different(F=23.005,P= 0.000),There was no significant difference between the sham-operation group and the blank control group(P=0.564).There was significant difference between the model blank group,the Yaotongning group and the Qiangjinzhuanggu group and the blank control group(P=0.000,0.002,0.001,0.000),the model blank group,the Yaotongning group and the Qiangjinzhuanggu heji group were significantly different(F=0.000,0.010,0.001)There were significant differences between the two groups(P=0.001 and 0.002,respectively).The expression of AQP5 was detected by RT-PCR in blank control group,sham operation group,model blank control group,Yaotongning group and Qiangjinzhuanggu heji group.The m RNA level of AQP5 in each group was significantly different(F=48.536,P=0.000,P=0.364).There was significant difference between the blank control group,the Yaotongning group and the Qiangjin Zhuanggu heji group and the blank control group(P=0.000,0.001,0.003,0.004 respectively).There was significant difference between the model group,the Yaotongning group and the Qiangjinzhuanggu hej group(P=0.000,0.012 and 0.001 of AQP3expression: After successful modeling,AQP3 positive area in the model blank control group compared with the blank control group and sham operation group(F= 8.232,P=0.007),except the blank control group and sham operation group There was no difference(P=0.771),the rest were statistically significant(P=0.042,P=0.033).Immunopositive substances concentrated in the cytoplasm,dark blue cells.The area of AQP5-positive cells in the blank control group was still significantly different from that in the Yaotongning and Qiangjin groups(F=4.332,P=0.012).There was a statistically significant difference between the two groups Significance(P= 0.091,P=0.000).Immunohistochemical detection of AQP5 expression: After successful modeling,AQP5 positive area in the model blank control group compared with the blank control group and sham operation group(F=9.603,P=0.006),except for the blank control group and sham operation group There was no difference(P=0.543),the rest were statistically significant(P=0.012,P=0.022).Immunopositive substances concentrated in the cytoplasm,dark blue cells.The area of AQP5 positive cells in the blank control group was still significantly different from that in the Yaotongning group and the Qiangjin group(F=7.763,P=0.037).There was a statistically significant difference between the two groups Significance(P=0.012,P=0.001).Clinical efficacy part: 1,Comparison of VAS scores among the three groups: VAS scores of Physiotherapy group,Physiotherapy + Qiangjin group and Physiotherapy plus low back painening group were significantly lower than before treatment(P<0.05).After 1 week and 1 month after treatment,the VAS scores of physiotherapy+ Qiangjin group and physiotherapy + Yaotongning group improved obviously(P<0.05),and after 3 months and 6 months of treatment,Physiotherapy group,physiotherapy + strong The score of VAS in the tendons group and physiotherapy +Yaotongning group improved significantly compared with the previous(P <0.05).The comparison between the two groups found that the VAS scores of Physiotherapy+Qiangjin group decreased to the lowest,followed by Physiotherapy + Yaotongning group,and the Physiotherapy group was the highest.The three groups were significantly different(P<0.05).JOA score comparison of three groups: The JOA score of Physiotherapy group,Physiotherapy+Qiangjin group and Physiotherapy +Yaotongning group improved obviously before and after treatment,the difference had significance(P<0.05).After 1 week of treatment,the JOA scores of physiotherapy+Qiangjin group and physiotherapy+Yaotongning group improved significantly(P<0.05)except for physiotherapy group.After treatment for 1 month,3months and 6 months,the Physiotherapy group,physiotherapy+Qiangjin group and physiotherapy+Yaotongning group improved compared with those of the former group(P <0.05).The comparison between the two groups showed that the JOA score of Physiotherapy+Qiangjin group increased the highest,followed by Physiotherapy+Yaotongning group,and lowest in physiotherapy group.After treatment for 3 months and 6 months,there was no significant difference between Physiotherapy + Qiangjin group and Physiotherapy+Yaotongning group(P>0.05).There was significant difference between the two groups(P<0.05).Comparison of relative signal intensity(RSI)of three groups of MRI nuclei: Through the research,it is found that the relative signal intensity of nucleus pulposus of intervertebral disc in the Physiotherapy group,physiotherapy + Qiangjin group and physiotherapy +Yaotongning group before and after treatment there are differences(P<0.05),indicating good curative effect of both groups.The comparison between groups showed that the relative signal intensity of MRI nucleus after Physiotherapy+Qiangjin group had the highest increase,followed by Physiotherapy+Yaotongning group,and the signal intensity of physiotherapy group had the lowest increase.The three groups were significantly different(P <0.05).Conclusion Degeneration of lumbar intervertebral disc is related to the expression of water channel protein.The regulation of AQP1,AQP3 and AQP5 by Qiangjin Zhuanggu Qufeng heji is one of its curative mechanism,which has the effect of improving lumbar disc degeneration.Qiangjin Zhuanggu Qufeng heji for degenerative diseases of lumbar obvious effect,the clinical effect of good use;the same time with the same type of traditional Chinese medicine preparation,its efficacy is more prominent.At the same time in the degenerative lumbar disc,the relative signal intensity of nucleus pulposus is related to the content of aquaporin.In clinic,the relative signal intensity of MRI nucleus can be measured to determine the degree of lumbar disc degeneration.
