| Objective:To investigate the main risk factors for stent failure in patients treated with a cystoscope retrograde ureteral stent for unilateral malignate ureteral obstruction,and establish the logistic regression model and evaluate the sensitivity,specificity and accuracy for predicting urteral sent failure.Methods:Through retrospectively analyzed the clinical date of 94 patients who required decompression of unilateral malignate ureteral obstruction by cystoscope retrograde ureteral stenting between January 2012 and December 2014.Statistical analyses were performed with SPSS ver.18.0 Statistical significance was set at P<0.05 for all analyses.First,univariate analysis was used in 21 clinical variables.Then multiplevariate logistic regression analysis was used in the seleted variables.At last the logistic regression equation was set up,the odds ratio was calculated:OR=Exp(B),and the sensitivity,specificity and accuracy of the model was evaluated individually.Results:A univariate analysis showed the significant predictors of stent failure to be serum albumin level,cause of obstruction,bladder invasion,CRF staging,level of hydronephrosis,whether to use superslip guide wire,retroperitoneal metastasis.By entering these factors into forward logistic regression analysis,it was revealed that there were four significant factors as serum albumin level,cause of obstruction,level of hydronephrosis,whether to use superslip guide wire.Logistic regression equation.With probability=0.5 as boundary,to figure out the comparative statement of predicted value with actual value,the sensitivity,specificity and accuracy of the logistic model were 86.5%,85.7%,86.2%.Conclusions:The main risk factors for stent failure in patients treated with a cystoscope retrograde ureteral stent for unilateral malignate ureteral obstruction were serum albumin level,cause of obstruction,level of hydronephrosis and whether to use superslip guide wire.The logistic regression model based on these factors can help urologists predict the failure rate of cystoscope retrograde ureteral stent for unilateral malignate ureteral obstruction. |
