Comparison Of The Applied Value Of Two Types Of Ureteralstents In The Treatment Of Ureteral Obstruction Caused By Malignant Tumors

Objective Comparative analysis of the efficacy,complications,indwelling time,and indwelling methods of ureteral stent with two different materials in the treatment of malignant ureteral obstruction(MUO)caused by malignant tumors,to guide clinical choices for more suitable treatment of ureteral obstruction caused by malignant tumors Ureteral stent,and discuss the management of complications related to ureteral stent.Method A retrospective analysis of 56 patients who underwent ureteral stents for MUO in the urology department of Xijing Hospital from January 2018 to December2019.According to the different stent materials,the patients were divided into metal ureter group and polyester ureteral stent group.Follow-up of the two groups of patients were analyzed respectively,postoperative complications,quality of life score(QOL),questionnaire for ureteral stent-related symptoms(USSQ),stent placement time,complications,number of stays,and hospitalization costs.Results The longest follow-up period was 18 months,of which 5 patients died(2in the metal stent group and 3 in the polyester stent group).The cause of death was tumor progression.The success rate of one tube placement in 21 patients in the metal stent group was greater than the success rate in one tube placement in 35 patients in the polyester stent group(P>0.05).Patients with unsuccessful first tube placement were placed in a double-lens position with oblique supine lithotomy,The metal composition power is 100%(16/16)greater than the polyester group 95%(19/20);the number of stent tube placements in the metal group is less than the polyester group(P<0.05);the average hospitalization cost of the metal group is greater than the polyester group(P<0.05);the retention time of patients in the metal stent group was longer than that in the polyester group(P<0.05);for the surgical effect,the pelvis separation of the two groups before surgery was similar to the polyester group(P>0.05);the metal stent group The resolution of renal pelvis was lower than that of polyester group in January and June(P<0.05);at 1 month after operation,the degree of remission of hydronephrosis was similar in both groups(P>0.05).At month,the degree of relief of hydrocephalus in the metal group was greater than that in thepolyester group(P<0.05);there was no significant difference in the quality of life score(QOL)between the two groups before surgery(P>0.05),and the postoperative quality of life score(QOL)metal stent The group was larger than the polyester group(P<0.05).The difference between the two groups was statistically significant;the results of the questionnaire on ureteral stent tube related symptoms(USSQ): the lower urinary tract symptom score metal group was not significantly different from the polyester group(P>0.05)The score of the metal group was lower than that of the polyester group in terms of pain related to stent placement(P<0.05),and the score of the metal group was lower than that of the polyester group(P<0.05)in terms of the effect on overall health.In the score,the metal group was lower than the polyester group(P<0.05).The difference between the two groups was statistically significant.The incidence of urinary tract irritation,hematuria,pain,and difficulty in removal was similar in the metal group after complications of stent placement,and the difference was not statistically significant.Significance(P>0.05);the incidence of infection,encrustation,and scaffold displacement in the metal group were lower than those in the polyester group(P<0.05),and the difference between the two groups was statistically significant.Conclusion 1.Patients with malignant ureteral obstruction often have no specific clinical manifestations in the early stage,which requires the attention of clinicians.2.The treatment effect of metal ureteral stent tube in the treatment of ureteral obstruction caused by malignant tumor is better than that of polyester ureteral stent tube,and the indwelling time is long.The incidence of complications is low,but the cost is more.It can be recommended for MUO patients.3.For patients with complicated malignant ureteral obstruction,we can actively choose oblique supine lithotomy double endoscopic combined with ureteral stent placement to improve the success rate of catheterization.
