Objective:In recent years, air pollution has caused great harm to people’s health.Among the acute malignant diseases,Cardiovascular disease and Cerebrovascular disease are common diseases that seriously threaten human health, especially the health of middle-aged and elderly people over 40 years old. They are characterized by high morbidity, high disability rate and high mortality. It is a common disease that poses a serious threat to human health, especially for middle-aged and elderly people over 40years old.It has the characteristics of high incidence, high disability rate, and high mortality rate.Previous studies have shown that there is a close correlation between fine particles such as PM2.5 and the mortality of people with cardiovascular and cerebrovascular diseases, while the research on the impact of gaseous pollutants on the mortality of cardiovascular and cerebrovascular diseases is insufficient. Therefore, this study analyzes the impact of gaseous pollutants on the mortality of cardiovascular and cerebrovascular diseases in Shenyang from 2014 to 2018,in order to further improve air quality, scientifically manage the atmospheric environment in Shenyang, and provide scientific basis for formulating relevant pollution prevention and control policies and laws and regulations. Methods:The death data of cardiovascular and cerebrovascular diseases in Shenyang from 2014 to 2018 were collected, with a total of 144911 people. The obtained air pollutant data included sulfur dioxide(SO2),nitrogen dioxide(NO2),carbon monoxide(CO)and ozone(O3).The meteorological data included daily average temperature, daily average pressure, daily average wind speed and relative humidity.Using a distributed lag nonlinear model based on Poisson distribution, multiple factors such as week effect, holiday effect,long-term factor, and meteorological factor are controlled in this model to analyze the impact of gaseous pollutants in the atmosphere on the mortality rate of cardiovascular and cerebrovascular diseases, and further study the impact of gaseous pollutants in the atmosphere on the mortality rate of cardiovascular and cerebrovascular diseases under different distribution characteristics such as season, gender, and age.Results:1.The annual average concentrations of SO2,NO2,CO and O3 in Shenyang from 2014 to 2018 were 46.62μg/m3,41.46μg/m3,0.98 mg/m3 and 58.75μg/m3respectively.The average temperature is 9.12℃,the relative humidity is 59.87%,the average air pressure is 1010.46Hpa,the average wind speed is 2.14km/h.the concentrations of SO2,NO2,CO and O3 have obvious seasonal variation trend. The concentrations of SO2,NO2 and CO are higher in cold season than in warm season, and the concentrations of O3 are higher in warm season than in cold season.2.The week effect, holiday effect,long-term factors, meteorological factors and other factors are controlled, and the distributed lag nonlinear model is used for analysis. When the concentration of SO2 increases by 10μg/m3,there is a harmful effect on the mortality rate of cardiovascular and cerebrovascular diseases in the total population at lag1.It reached the maximum at the concentration of 500μg/m3 in lag3-lag4,and the RR value was 1.12(95%CI:1.09-1.48).When the concentration of NO2 rises by 10μg/m3,the mortality of cardiovascular and cerebrovascular diseases in the total population has a harmful effect on lag1 day,and reaches the maximum in lag3 when the concentration is120μg/m3,and the RR value at this time is 1.08(95%CI:1.05-1.35).When the CO concentration increases by 0.3mg/m3,the mortality rate of cardiovascular and cerebrovascular diseases has a harmful effect on lag2,and the highest concentration is at lag3,with the strongest lag effect, and the greatest risk is at 3mg/m3,when the RR value is 1.04(95%CI:1.03-1.33).When the concentration of O3 increased by 10μg/m3,the mortality of cardiovascular and cerebrovascular diseases in the total population had a harmful effect at lag1,and reached the maximum at lag1 and at the concentration of250μg/m3,and the RR value was 1.10(95%CI:1.04-1.46).3.In different seasons, the RR value of SO2 in cold season is higher than that in warm season, and the RR value of NO2,CO and O3 in cold season is lower than that in warm season.Under different genders, the RR values of SO2,NO2,CO and O3 are higher in women than in men.At different ages, the RR values of SO2,NO2 and CO were higher in the elderly group than in the non-elderly group. There was no lag effect in O3 non-elderly group.Conclusion:1.SO2,NO2,CO,O3 are related to mortality of cardio-cerebrovascular diseases in Shenyang, and there is a lag effect.2.The influence of SO2,NO2,CO and O3 on the mortality of cardio-cerebrovascular diseases in Shenyang has seasonal differences, gender differences and age differences. The influence of SO2 is more significant in cold season, and the influence of NO2,CO and O3 is more significant in warm season. The influence of SO2,NO2,CO and O3 on women is greater than that of men,and the influence on the elderly is greater than that of the non-elderly.
