Ojective:To analyze the impact of Changchun City atmospheric pollutants on the burden of cardiovascular disease in residents.To understand the air quality status of Changchun City and the time distribution of residents of cardiovascular and cerebrovascular diseases.To screen out the population sensitive to pollutants by gender and age.To analyze the effect of pollutants in different seasons by season.Analyz the effect of the combined effect of various pollutants on the life loss years of cardiovascular and cerebrovascular diseases.Methods:Collecting Changchun City 2018-2020 atmospheric pollution data,meteorological data,death caused by death.Using death life loss as a disease burden indicator,calculate the life of the residents of cardiovascular and cerebrovascular diseases,the year.Using the time-layered case cross design combined with the condition Poisson regression.Evaluating the impact of the annual loss of air pollution on the losses of the life of cardiovascular and cerebrovascular disease in residents.Results:1.During 2018 to 2020,Changchun City Atmospheric Pollutants PM2.5,PM10,SO2,NO2,O3,CO daily concentration were 37.57μg/m~3,63.75μg/m~3,12.23μg/m~3,33.26μg/m~3,54.64μg/m~3,0.75 mg/m~3.The main pollutants were PM2.5,PM10.The temperature of the air mass index AQI is 62.66,and the air quality is excellent in537 days,accounting for 49.00%.Atmospheric pollutant concentration has seasonal changes,the concentration of winter pollutants is higher,the summer concentration is low(except O3).O3 presents a relatively high temperature concentration in winter and a relatively high summer concentration.2.During the study,Changchun City was a total of 78,268 cases of cardiovascular and cerebrovascular disease,57.43%of total deaths.The years of life loss caused by the death of cardiovascular and cerebrovascular diseases totaled 1304048.92 years.The average cardiovascular disease death was 71.41 people/day,and the average daily cardiovascular disease death YLL was 1189.83 years/day.Residents’heart cerebrovascular system disease Spring and winter season 42305 deaths,accounting for52.77%of the annual cardiovascular disease death,36,963 deaths in summer and autumn,accounting for 47.23%of the annual cardiovascular disease death.The cyclophosphate disease death YLL was higher in January and March,respectively,from128633.09/Day,118064.22 years/day.3.Correlation analysis.The correlation between PM2.5 and PM10,SO2,NO2,and CO have significant statistical significance(P<0.01).The correlation coefficient was up to PM2.5 and PM10(r=0.871,P<0.01).In meteorological factors,the correlation between temperature,humidity,and air pressure and contaminant concentration has significant statistically significant significance(P<0.01).The meteorological factors(except air pressure)and contaminant concentration were mostly negatively correlated.There is a significant linearity between the pollutants.The correlation between PM2.5,PM10,SO2 and cardiovascular and cerebrovascular disease YLL was statistically significant(P<0.01).4.The results of the single pollutant model showed that the effect of short-term exposure to PM2.5,PM10,NO2,O3,and CO on YLL of cardiovascular and cerebrovascular disease in residents was statistically significant.PM2.5.PM10,NO2 and CO had the greatest effect on YLL of cardiovascular and cerebrovascular diseases when they lagged behind for 2 days.The RR values were RR=1.008(95%CI:1.003-1.012),RR=1.006(95%CI:1.003-1.010),RR=1.014(95%CI:1.001-1.027),RR=1.083(95%CI:1.027-1.141).O3 had the greatest effect on YLL of cardiovascular and cerebrovascular diseases on the lag day(RR=1.010,95%CI:1.003-1.017).The effect of SO2 on YLL of cardiovascular and cerebrovascular diseases was not statistically significant.5.According to gender layering,PM2.5,PM10,O3,CO of different gender groups have statistically significant impact on residents’cardiovascular and cerebrovascular disease YLL,but the best lag period is different.In men’s grouping,the best stagnation periods of PM2.5,PM10,O3 and CO are lagging behind for 3 days.In women’s grouping,the best stagnation of air pollutants is lagging behind for 3 days,3 days of accumulation,2 days behind,and 2 days behind.6.According to age groups,The effect of PM2.5,PM10,O3,and CO in the age group of<65 years on YLL of cardiovascular and cerebrovascular diseases was statistically significant,and the optimal stagnation period is different.Both the best lags of air pollutants PM2.5,PM10,CO are 2 days behind,and the best stagnation periods of O3 is lagging behind 1 day.The effect of PM2.5,PM10,SO2,O3,CO in the age group≥65 years on YLL of cardiovascular and cerebrovascular diseases were statistically significant.The optimal lag time of pollutants is 2 days,the day of lag,4 days of cumulative lag,the day of lag and 2 days of lag.7.According to the seasonal layers,the influence of PM2.5,PM10,O3,COin the spring and winter season is statistically significant.The best lag periods are 2 days later,the same day later,7 days later and 2 days later.The effects of PM2.5,PM10,SO2,NO2,O3,and CO have statistically significant significance on the YLL of cardiovascular and cerebrovascular diseases.The optimal lag time of pollutants are lag day,cumulative lag3 days,cumulative lag 6 days,cumulative lag 7 days,lag 4 days and lag 5 days respectively.8.There is a combined effect of air pollutants on YLL of cardiovascular and cerebrovascular diseases.In the multi-pollutant model,PM2.5 and SO2,NO2,CO,O3have the greatest effect.The concentration of PM2.5 increased by 10μg/m3,the RR value affected by the residential cerebrovascular disease YLL was 1.013(95%CI:1.005-1.021),which was significantly increased compared to the PM2.5 single contaminant model.The pollutant model of PM10 and SO2,NO2,CO,O3 was the largest,and the concentration of PM10 was increased by 10μg/m3,and the RR value affected by the human cerebrovascular disease YLL was 1.011(95%CI:1.005-1.017).The combination of NO2 and SO2 and O3 is greater than simple NO2(RR=1.023,95%CI:1.008-1.038).The combination of CO and NO2,SO2,O3 is greater than simple CO(RR=1.161,95%CI:1.048-1.285).Conclusion:1.The air quality of Changchun is in good condition from 2018 to 2020.And the primary pollutant mainly is PM10,PM2.5.The concentration of atmospheric pollutants varies from the season.The concentration of pollutants is higher in winter,and the summer is lower(except O3).O3 has a low concentration trend in winter,and the summer concentration is relatively high.2.Short-term exposure to PM2.5,PM10,NO2,O3,CO and residential cerebrovascular disease YLL presence statistical association.The effects of contaminants on YLL of residents’cardiovascular diseases have lagging effect,and the health effects of residents have reached the maximum within 0-4 days.3.Atmospheric pollutants have different effects on the YLL of the heart and cerebrovascular disease of different gender and age,and have seasonal differences at the same time.Women are more sensitive to O3 and CO.The residents of≥65 are more sensitive to PM10,PM2.5,O3.And the<65-year-old group is more sensitive to CO.Residents in spring and winter are more sensitive to PM10,PM2.5,and CO,and PM10have a stronger effect.Residents in summer and autumn are more sensitive to O3,and the effect value of PM2.5 is more effective.4.There is a combined effect of air pollutants on YLL of cardiovascular and cerebrovascular diseases.
