In digitalization era,the role of information system is not only to help organization to improve efficiency and lower cost, but more important is to help to make decision fast and accurately and reshape organization's value chain. Therefore, the successful application of information system plays an important role in whether the organization can successfully complete digital transformations. In recent years, organizations have continued to increase their investment in information system, but the overall success rate is low.And some failures even threat the survival of the organization. Therefore,it is of great practical significance to study why the information system fails, and to learn from the failures and experience,so as to improve the success rate.Based on the previous theories in literature, this paper sets up an HLM model by using the internal development and customer's using data of one information system of company S,also interviews relevant specialists to collect feedbacks from more comprehensive perspective. And then find out the reasons which lead to system fails through analyzing the regression model and interview results, and then get some lesson learned as well. The conclusions from the analysis of modeling and interview results are that the system use has a significant impact on the lifecycle of the system. The deeper the user uses, the more positive the impact;the more functions the system develops and the more R&D investment, the poor quality of the system and the inability to improve system quality through large technical changes(such as code refactoring);system quality has no significant impact on the lifecycle of the system during the initial use phase. All these conclusions and lessons are valuable references and guidance for S company's subsequent product development activities.
