Research On Social Work's Participation In Rehabilitation Services For Elderly Patients With Mental Disorders Under The Mode Of Combination Of Medical And Nursing Care

The aging of the population in China is a serious challenge,the number of elderly population continues to increase.In order to solve the problem of medical care and old-age care for the elderly,China's 14 th Five-Year Plan proposes to "foster a new business model for old-age care,and build a old-age service system that is coordinated between home and community institutions and combines medical care with health care." The mode of combining medical care and nursing has been developed continuously under the guidance of policy support.Shenyang is an old industrial base,aging society is under great pressure.The 14 th Five-Year Plan of Shenyang City points out that we should promote the health pension industry and cultivate the new business form of the pension industry."Establishment of medical and nursing institutions for the disabled,semi-disabled,mentally disabled and elderly people." Shenyang area now has 37 pilot institutions combining medical and nursing care,and 13 pilot institutions of hospice care.In order to promote the standardization and specialization of the mode of combining medical care with medical care,it is necessary to attract social forces to actively participate in the construction of the service system of combining medical care with medical care.The background of the innovative development of the mode of combining medical care and nursing care provides necessary conditions and support for social work to participate in the rehabilitation services of elderly patients with mental disorders.Shenyang A Hospital is A third-level psychiatric hospital of integrated Chinese and Western medicine,and one of the first batch of dual-pilot hospitals of state-level combination of medical care and hospice care.In the "combination of medical and nursing beds" mode of continuous innovation,the development of integrated traditional Chinese and western medicine,psychological rehabilitation,nursing care integration service mode.Since the establishment of the medical care center and geriatric ward in 2018,it has served more than 1,000 disabled and semi-disabled elderly people with mental disorders.Based on my internship experience in the geriatric ward of combining medical care with nursing care,I got familiar with the working process of combining medical care with nursing care in the treatment area through participatory observation.According to the comprehensive condition assessment of patients and psychosocial scale assessment,it is found that elderly patients with mental disorders have many and complex syndromes,are troubled by physical diseases and emotional and mental disorders,and have needs for medical care,respect,emotional support and interpersonal communication.The treatment area of geriatric ward mainly provides medical treatment and nursing services,supplemented by rehabilitation services.The difficult problems of combining medical care with rehabilitation care services are: first,the lack of professional care talents for elderly patients;second,the boring content of cultural and entertainment life;third,the lack of participation and support from family members in rehabilitation services.The combination of medical care and nursing care is an innovation of service mode,which is inclusive and innovative and integrates the advantages of various specialties.It is feasible and necessary for social work to participate in the rehabilitation service of elderly patients with mental disorders under the mode of combining medical care with nursing care.Under the guidance of the theory of cognitive behavioral therapy to carry out "bipolar disorder" case care services.It changed the negative mood of the client and relieved the depressive symptoms.Carry out recreational group activities for the elderly to improve the mental state and interpersonal relationship of the group members.In order to improve the effect of rehabilitation service,it is necessary to promote the construction of the rehabilitation service system combining medical care with nursing care.On the one hand,we should use a variety of working methods,pay attention to the existing and explore potential service objects,and improve the breadth,width and depth of social work participation services.On the other hand,we should clearly define the position of social work in rehabilitation services,pay attention to the application of communication skills,improve the sense of self-efficacy of elderly patients,and promote the construction of family support system for elderly patients.
