Research On The Status Quo And Influencing Factors Of Parental Stress In Children With Cleft Lip And Palate

Objective:To investigate parenting stress and influencing factors of the parents of patients with cleft lip and palate,so as to provide basis for improving the parenting stress of the parents.Methods:This is a descriptive study.A questionnaire survey was conducted among 152 paren-ts of patients with cleft lip and palate who met the admission and discharge standards in Two tertiary grade A stomatological hospitals in China by convenience sampling met-hod.The research tools included General Information Questionnaire,Parenting Stress Inde-x Short Form,Family Environment Scale-Chinese Version,Generalized Anxiety Disorder an-d Perceived Social Support Scale.SPSS 20.0 software was used for statistical analysis.The general data of children with cleft lip and palate and their parents were analyzed by frequency and composition ratio.Parents'parental stress,family environment,anxiety and perceived social support were statistically described by means and standard deviati-on.The difference of different demographic data in the total score of parental stress in children with cleft lip and palate was analyzed by T test or one-way variance analysis.Pearson correlation analysis was used to analyze the correlation between parents'stress and family environment,anxiety and perceived social support.The influencing factors of parental stress in children with cleft lip and palate were analyzed by multiple linear reg-ression analysis.Results:1.The parenting stress of parents of patients with cleft lip and palate:the total scor-e was(90.35±16.80)points.The scores of each dimension:PD dimension was(32.64±7.34)points,PCDI dimension was(26.76±6.93)points and DC dimension was(30.95±6.72)points.The total score of family environment was(50.53±8.28)points.The scores of each dimension:cohesion was(7.38±1.85)points,expressiveness was(5.55±1.52)points,conflict was(2.64±2.11)points,independence was(5.64±1.28)points,achievement was(5.09±1.38)points,intellectual cultural orientation was(4.53±2.07)points,active recreatio-nal orientation was(3.77±2.57)points,moral religious emphasis was(6.41±1.65)point-s,organization was(6.67±1.74)points and control was(3.93±2.01)points.The total sco-re of parents'anxiety was(4.32±4.35)points.The incidence of anxiety was 39.5%,and the incidence rate was 39.5%,of which 28.9%were mild,5.9%were moderate,and 4.7%were severe.The total score of social support was(62.74±12.36)points.The scores of each dimension:domestic support was(21.68±4.65)points,friend support was(20.17±4..96)points and other support was(20.88±4.29)points.2.Univariate analysis results:Children's age,genetic history,parents'age,parents'working status,family location,family average monthly income,and whether they have received surgical treatment had statistical significance on the parenting stress of parents of patients with cleft lip and palate(P<0.05).3.Results of relevance analysis:The total scores of parenting stress of parents were Passively correlated with the total scores of cohesion,active recreational orientation,organization,social support and their dimensions(P<0.05),and energetically correlated with conflict,independence and the total scores of anxiety(P<0.05).4.Multivariate analysis results:Genetic history,active recreational orientation,indepen-dence,friend support and anxiety were the factors of parenting stress of parents of pati-ents with cleft lip and palate.After adjustment R~2=0.457,it could explain 45.7%of the variation in parenting stress.Conclusion:1.The parenting stress of parents of patients with cleft lip and palate is at a critical high level,and the highest score dimension is parental distress.Parents'overall understanding of social support level is medium to upper and the incidence of anxiety is high.The family atmosphere of children with cleft lip and palate is depressed,showing the characteristics of low cohesion,high contradiction,high emphasis on ethics and values,and more use of fixed procedures to arrange life;2.The active recreational orientation and independence of family environment,parents'anxiety,friends'support in understanding social support,parents'age and whether children have genetic history have important influence on parents'job stress.That is to say,the parents of children with cleft lip and palate who have low active recreational orientation level,high independence level,high parents'age,high anxiety level,low friend support and children with genetic history have relatively high parenting stress level.
