| Purpose: In this study,we established gentamicin induced ototoxicity and nephrotoxicity rat models,and observed the effects of three prevention and treatment methods,namely,benign electroacupuncture stimulation of Sanjiao meridians,treatment from kidney,acupuncture and medicine,on the morphology of cochlea and kidney tissue,the contents of tumor necrosis factor-?(TNF-?),monocyte chemoattractant protein-1(MCP-1),macrophage inflammatory protein-2(MIP-2),serum creatinine(Scr)and urea nitrogen(BUN)in renal tissue of gentamicin induced auricle injury rats were studied,objective to explore the possible mechanism of acupuncture combined with medicine on gentamicin induced ototoxicity and nephrotoxicity in rats.Material and method: Forty healthy male SD rats were randomly divided into blank group,model group,electroacupuncture group,traditional Chinese medicine group(TCM group)and combined acupuncture and traditional Chinese medicine group(combination group),with 8 rats in each group,for ABR test after adaptive feeding for one week.The blank group was intramuscular injected with normal saline for 14 days,once a day,without treatment.Model group GM(120mg/kg)was intramuscular injected for 14 days,once a day,without treatment.The dose and method of GM in the electroacupuncture group,the TCM group and the combination group were the same as those in the model group.At the same time,the electroacupuncture group was given acupuncture at Yifeng,Waiguan and Yangchi points on both sides every day.After acupuncture,the electroacupuncture was added and the needle was retained for 20 minutes,alternately on both sides,once a day,for 14 consecutive days.In the TCM group,rats were given Jingui Shenqi Pill intragastric gavage at the same time,5ml/kg/d,once a day for 14 days.The combination group were given electroacupuncture group and TCM group intervention measures.After treatment,all rats were sacrificed after ABR test again.The content of TNF-?,MCP-1,MIP-2 in kidney tissue were detected by enzyme-linked immunosorbent assay.Scr and BUN in serum were detected by automatic biochemical analyzer.Results:1.The results of ABR hearing threshold test showed that: before the beginning of the experiment,ABR thresholds of the five groups were lower,and the difference was not statistically significant(P>0.05).Comparison after 14 days of experiment: compared with blank group,ABR threshold in model group,electroacupuncture group and TCM group increased(P <0.01),and the improvement was more obvious in combination group(P<0.05).Compared with model group,ABR threshold of TCM group was decreased(P<0.05),and that of electroacupuncture group and combination group was more obvious(P<0.01).Compared with the TCM group,the ABR threshold of the electroacupuncture group was slightly lower,but there was no significant difference between the two groups.Compared with the electroacupuncture group and the TCM group,the decrease amplitude of the combination group was more significant(P<0.01).2.He staining results showed that:To observe the pathological changes of cochlea tissue: In blank group,the cochlea tissue morphology was normal,the spiral ganglion morphology and structure were complete,the number and morphology of nerve cells were normal,and the hair cells were neatly distributed and arranged.Compared with the blank group,the morphological structure of the cochlea tissue in the model group was seriously damaged,and the spiral ganglion had obvious pathological changes.The number of nerve cells was missing,the boundary was blurred,and the hair cells were missing obviously and arranged disorderly.Compared with the model group,the morphological and structural damage of cochlea tissue in the TCM group,the electroacupuncture group and the combination group were all reduced,the pathological changes of the spiral ganglion were reduced,the number of cells was a little missing,the boundary was clear,the number of hair cells was less lost,and the arrangement was orderly,and the improvement effect of the combination group was more significant.Observed the pathological changes of renal tissue: The renal tissue of the blank group was normal,and the glomerulus and tubules were intact.Compared with the blank group,the model group showed obvious renal damage,abnormal renal tubular structure,and vacuoles of renal tubular epithelial cells were found,and the infiltration of inflammatory cells in renal interstitial cells was obvious;Compared with the model group,the phenomenon of kidney injury in the electroacupuncture group,the TCM medicine group and the combination group were all reduced to a certain extent,and the infiltration of inflammatory cells was reduced,and the improvement effect in the combination group was more significant.3.The results of ELISA showed that:The content of TNF-? in rat kidney was as follows: Compared with the blank group,the content of TNF-? in the kidney tissue of the model group,the electroacupuncture group,the TCM group and the combination group was increased(P<0.01);compared with the model group,the content of TNF-? in the electroacupuncture group,the TCM group and the combination group was decreased(P<0.01);compared with the electroacupuncture group,the content of TNF-? in the TCM group was slightly lower than that in the electroacupuncture group,but the difference was not significant;compared with the electroacupuncture group and TCM group,the decrease in the combination group was more obvious(P<0.01).The content of MCP-1 in rat kidney was as follows: Compared with blank group,the content of MCP-1 in renal tissue of rats in model group,electroacupuncture group and TCM group was increased(P<0.01),and the improvement was more obvious in combination group(P<0.05).Compared with model group,the content of MCP-1 in electroacupuncture group,TCM group and combination group decreased(P<0.01);Compared with the electroacupuncture group,the content of MCP-1 in the TCM group was decreased(P<0.05),and the decrease was more obvious in the combination group(P<0.01).Compared with the TCM group,the decrease in the combination group was more significant(P<0.01).The content of MIP-2 in rat kidney was as follows: Compared with the blank group,the content of MIP-2 in the kidney tissue of the model group,the electroacupuncture group,the TCM group and the combination group was increased(P<0.01);compared with the model group,the content of MIP-2 in the electroacupuncture group,the TCM group and the combination group was decreased(P<0.01);compared with the electroacupuncture group,the content of MIP-2 in the TCM group and the combination group decreased significantly(P<0.01);compared with the TCM group,the decrease in the combination group was more obvious(P<0.01).4.The results of serum Scr and BUN level showed that:Serum SCR level was as follows: Compared with blank group,serum Scr level in model group,electroacupuncture group,TCM group and combination group were all increased(P<0.01).Compared with model group,Scr level in electroacupuncture group was decreased(P<0.05),and the decrease was more obvious in TCM group and combination group(P<0.01).Compared with electroacupuncture group,Scr level in TCM group and combination group was significantly decreased(P < 0.01).Compared with the TCM group,the level of combination group decreased more significantly(P<0.01).BUN level in serum was as follows: Compared with the blank group,the level of BUN in the model group,the electroacupuncture group,the TCM group and the combination group was increased(P < 0.01);compared with the model group,the level of BUN in the electroacupuncture group was lower than that in the model group,but there was no significant difference between the two groups;compared with the electroacupuncture group,the level of BUN in the TCM group and the combination group were significantly decreased(P<0.01);compared with the TCM group,the decrease in the combination group was more obvious(P<0.01).Conclusion:1.Gentamicin has both ototoxicity and nephrotoxicity,which can successfully induce ototoxicity and nephrotoxicity in rats and establish a model of ear and kidney injury.2.The benign electroacupuncture stimulation of Sanjiao meridians can reduce the ototoxicity and nephrotoxicity induced by gentamicin in rats,and can enhance the prevention and treatment effect of prescriptions based on kidney.3.The combination of acupuncture and medicine based on the theory of "kidney and Sanjiao are interlinked" can play a good role in the prevention and treatment of gentamicin induced renal dysfunction,can significantly reduce the content of inflammatory factors in kidney tissue,reduce kidney injury,and effectively improve the hearing loss of rats.4.It is verified that acupuncture and medicine simultaneously play an important role in the prevention and treatment of drug-induced deafness. |
