Research On Hierarchical Diagnosis And Treatment System From The Perspective Of Social Trust Theory

With the rapid development of China’s economy and society,people’s living standards have gradually improved,and the public has paid more and more attention to their own health.Community health service center,as a city non-profit health service organization undertaken by the government,not only undertakes daily outpatient work,but also shoulders multiple missions such as maternal and child health care,vaccination,chronic disease management and public health prevention.In September2015,the General Office of the State Council issued"Guiding Opinions on Promoting the Construction of Graded Diagnosis and Treatment System",which pointed out that the establishment of graded diagnosis and treatment system is an important measure to rationally allocate medical resources and promote the equalization of basic medical and health services,is an important content to deepen the reform of medical and health system and establish a basic medical system with Chinese characteristics,and is of great significance for promoting the long-term healthy development of medical and health undertakings,improving people’s health level and ensuring and improving people’s livelihood.After more than ten years of system practice,the implementation effect of graded diagnosis and treatment system has initially appeared.However,in some areas,due to the relatively weak trust between doctors and patients in the community,there are still problems such as excessive pressure of diagnosis and treatment in tertiary hospitals and idle medical resources in community health service centers.It is urgent to build systematic social trust and reach a consensus on trust between doctors and patients in urban communities,so as to promote the efficient implementation of graded diagnosis and treatment system in urban communities in the new era.The report of the 19^th National Congress of the Communist Party of China pointed out that the main social contradictions in China have been transformed into the contradiction between the people’s growing needs for a better life and unbalanced and inadequate development.Therefore,social science research should adhere to the people as the main body and take the people’s longing for a better life as the goal.By establishing social trust,forming doctor-patient linkage,reducing social risk factors between doctors and patients,and forming a doctor-patient trust community;At the same time,based on the social ecological environment of the Internet,we should expand the way of creating and maintaining doctor-patient relationship,realize the synchronous development of doctor-patient trust community in physical space and cyberspace,and build a harmonious and stable new doctor-patient relationship in urban communities.In this paper,through in-depth field survey of W community health service center in Harbin,based on the summary of existing research results,based on the perspective of social trust theory,in-depth mining classification diagnosis and treatment system in the operation process of our country’s urban community health service center there are medical resources and facilities lag,"medical body"structure machinery is loose,medical hardware facilities update lag,community health care talent shortage,information-based medical services to carry out insufficient and other practical problems.These problems directly lead to the reduction of the frequency of doctor-patient interaction in urban communities,the weakening of the timeliness of doctor-patient communication,the increase of the tension between doctors and patients,and the reduction of the degree of trust between doctors and patients.They form a series of negative development factors of doctor-patient relationship and hinder the high-quality development of the new doctor-patient relationship in urban communities.Therefore,base on that analysis of the reasons for the formation of the political system,economic benefit,social culture and other problems,the existing problems and cause of the grading diagnosis and treatment system in china are clarified,how the doctor-patient relationship in urban communities changes from trust weakening to trust consensus is explored,how to effectively construct the doctor-patient trust community with the characteristics of social system are explored,and the development experience of the grading diagnosis and treatment system in three typical provinces and cities such as shanghai,zhejiang and fujian in china is used for reference,It is proposed to speed up the efficient implementation of the national graded diagnosis and treatment policy system,tap the advantages of community health service centers,enhance the mutual trust mechanism between community medical institutions,doctors and patients,and innovate the new urban community health system of"internet+medical",so as to optimize the healthy development of the graded diagnosis and treatment system in China,enhance the satisfaction degree of doctor-patient relationship in urban communities,relieve the pressure of diagnosis and treatment in tertiary hospitals,and promote the efficient implementation of the graded diagnosis and treatment system in China during the"14th Five-Year Plan"period,so as to provide reasonable policy enlightenment for further promoting the national strategy of"Healthy China 2030".
