Objective: To review the discrepancy of clinical effects and adverse reactions between Shengyang Yiwei prescription integrated with western medicine and pure western medicine in the therapy of AS,so as to explore and expand a secure and effective therapeutic regimen for AS.Methods: 60 patients who conformed to the standard of diagnosis were randomly split into two groups,30 cases in the control group and others in the experimental group.The control group was cured with thalidomide 50 mg qn and diclofenac sodium sustained release capsule 50 mg qn,while the experimental group was cured with Shengyang Yiwei prescription on the basis of the therapy12 weeks.The improvement of TCM syndrome score、ASDAS、BASDAI、BASFI、PGA、VAS score of spinal pain、ESR、CRP、finger groud distance、occipital wall distance、Schober test before and after therapy were analyzed.The routine examination of blood and urine,liver and kidney function were reexamined before and after treatment and at the 4th and 8th week of therapy to review the safety of treatment,and the adverse reactions during therapy were recorded good response.Results:1.After treatment,the effective rates of TCM symptom improvement in the two groups were 86.7% and 60% respectively,and the control and significant rates were23.3% and 3.3% respectively,the compliance rates of ASAS40 were 43.3% and16.7%,the differences were statistically significant(p<0.05).The compliance rates of ASAS20 were 73.3% and 53.3%,respectively,with no statistical difference(p>0.05).2.Intra group anslysis: there were significant differences in ASDAS-CRP 、BASDAI、BASFI、PGA、VAS score of spinal pain、ESR、CRP、finger groud distance、occipital wall distance、Schober test and total score of TCM syndrome between the two groups before and after treatment(p<0.05).Among the TCM symptom scores,the symptom of the experimental group were statistically significant(p<0.05).In the control group,there was no statistical difference in the improvement of abnormal stool、fatigue、limb sleepiness、abdominal pain and abdominal distension scores(p>0.05);for the symptom scores of lumbar pain 、limited activity、morning stiffness and night pain the difference was statistically significant(p<0.05).3.Comparison between groups: after treatment,there was no significant difference in ESR、CRP、finger groud distance、occipital wall distance、lumbar pain、activity limitation、morning stiffness、nocturnal pain symptom score and ASAS20 compliance rate between the two groups(p > 0.05).In addition the experimental group actually had statistical significance in ASDAS、BASDAI、BASFI、PGA、VAS score of spinal pain、Schober test、ASAS40 compliance rate、TCM effective rate、abnormal stool、fatigue、limb sleepiness、abdominal distension and abdominal pain(p<0.05).4.In the adverse reaction rate,the treatment group and the control group were26.7% and 6.7%,the discrepancy was obviously significant.Conclusion:1.The two groups can improve ASAS20 compliance rate、ESR、CRP、finger to ground distance、occipital wall distance、lumbar pain、limited activity、morning stiffness、nocturnal pain symptom score indicators,there is no significant difference in the degree of improvement.2.The treatment group was superior to the control group in the increase of ASAS40 compliance rate、TCM syndrome efficiency、ASDAS-CRP、BASDAI、BASFI、PGA、VAS score of spinal pain、Schober test、abnormal stool、fatigue、limb sleepiness、 abdominal pain and abdominal distension.3.Modified Shengyang Yiwei decoction combined with western medicine in the treatment of as can effectively improve the clinical efficacy and reduce adverse reactions,which can be used for clinical reference.
