Objective:In order to investigate the relationship between exercise ability and osteoporosis,and the influence of traditional Chinese medicine Compound Jin-Gu-Jian(CJGJ)on the exercise ability and bone metabolic marker in primary osteoporosis,the basic information collection and clinical observation were performed on the primary osteoporosis patients who were enrolled in clinic.All the work mentioned above provides helpful experience and new method for the prevention and treatment of primary osteoporosis with medications.Methods:According to the diagnostic criterion of primary osteoporosis,90 patients including outpatients and inpatients were collected in Xiyuan hospital).All the patients were randomly distributed to an treatment group and a contradistinguishing group with a proportion of 2:1.Same basic treatment were implemented in the two groups with one piece of CaCO3 D3 tablet and one calcitriol capsule daily.Besides the basic treatment,the two groups were carried out additional treatment both.The observing group was provided with CJGJ 2 times a day,and 200ml once.The CJGJ contains broken nodules,Eucommia,Pulmonaria,Poria,Achyranthes codonopsis codonopsis Atractylodes,etc.In the contradistinguishing group,alendronate sodium was provided weekly with a one-off dose of 70mg.Taking 3 months as one course of treatment,the Table of Clinical Observation on Osteoporosi was filled before and after the treatment,which containing lots of information,including the basic information of the patients,bone density,serum bone metabolic markers,grip strength,5 times of sitting time,3m returning time,falling distance of reacting ruler,standing time with one leg,etc.,The effect of drugs acting on muscle function and bone metabolism index was observed during the treatment.The correlation between exercise-related factors and bone density was also analyzed.Results:1.There were no statistical differences in baseline data such as sex,age,bone mineral density and athletic ability before treatment(P>0.05).2.The correlation coefficient between grip strength and femoral BMD was(0<r<0.5,P<0.05)after statistically analyzing,which appeared as a positive correlation.The correlation coefficient of 5 sitting time,3m reentry time and lumbar vertebrae,femur average T value and BMD value(-0.5<r<0,P<0.05)were negatively correlated.There was no correlation between the falling distance of reacting ruler,standing time with one leg and bone density.The correlation coefficient between BMI and femur T and BMD values(0<r<0.5,P<0.05)was positively correlated.3.After treatment,the grip strength of the treatment group increased significantly,the time for sitting up for 5 times and the time for 3m reentry were all significantly decreased;the grip strength of the control group showed no significant changes,and the time for sitting 5 times and the time for 3m reentry decreased significantly;There was no significant change in distance and erection time.There was no significant difference between the two groups after treatment.4.After treatment,the bone metabolic markers tPINP and osteocalcin were significantly increased in the treatment group,with significant difference(P<0.01),and little effect on?-CTX,and there was no statistical difference(P>0.05).Contradistinguishing group tPINP,osteocalcin changes little,no statistical difference(P>0.05),?-CTX decreased significantly,there is a significant difference(P<0.01).After treatment,?-CTX was significantly lower in the control group than in the treatment group(P<0.05).Conclusions:1.The incidence of primary osteoporosis disease are increased with ages,and the incidence of females is higher than that of males.2.CJGJ is effective for the primary spleen and kidney deficiency and blood stasis type of primary osteoporosis and is more safe.After treatment,the content of tPINP and osteocalcin in the serum concentration was increased for the treatment group,but the content of ?-CTX has no significantly changes,which suggesting that Chinese medicine was useful in promoting bone formation.In addition,there was a significant change in grip strength,5 times of sitting time,and 3m reentry time of patients for the treatment group after treatment.There were also significantly improvement trend for the descending distance of the feet and the erection time of the single leg.All the above demonstrate that CJGJ can not only alleviates the pain in the lower limbs of the patients,but also has a good effect in enhancing muscle function and exercise ability of the patients.3.BMI,grip strength,5 sitting time,3m reentry time are all related to the bone density,and these indicators can reflect the body's exercise ability.Chinese medicine believes that the spleen is mainly composed of limbs and muscles.If the spleen deficiency and transport are abnormal and the water valley is not transported,the muscles will not be rich,the muscle strength will be decreased and the bone nutrition will be lacking,which resulting in osteoporosis.All those also prove that spleen deficiency is a promoting factor of osteoporosis.Therefore,the effective methods for preventing and treating osteoporosis are maintaining spleen and stomach function while strengthening muscle function exercise.
