ObjectiveTo study the characteristics of insomnia of college students and the distribution law of TCM syndrome types, and analyze the correlation between TCM syndrome types of insomnia and indexes such as Heart Rate Variability,so as to provide the relevant theoretical basis for TCM syndrome differentiation of college students with insomnia.MethodsIn this study, college students in Beijing University of Chinese Medicine from September 2020 to December 2020 were selected as the research subjects in the form of voluntary participation and required to meet the inclusion criteria.A total of 124 insomnia subjects were collected. This research project has passed the ethical review by the Medical Ethics Committee of our university and obtained the review approval document(Proj ectN o.:2020BZYLL120).First of all, they recorded and completed the insomnia questionnaire(general situation, summary of current medical history, basic status, head and face symptoms, chest and abdomen symptoms, fecal status, tongue and pulse),Pittsburgh Sleep Quality Index(PSQI)and Self-Rating Sleep Status Scale(SRSS).Then, they received the non-contact heart rate and respiration recorder(model:VSM-M01,which has obtained the national medical device registration certificate)provided by Beijing Jusai Technology Development Co.,Ltd.for sleep monitoring and physiological data acquisition.According to the questionnaire of traditional chinese medicine(TCM)syndromes of insomnia,124 patients with insomnia were classified according to TCM syndrome differentiation, and the database was established for data management. The distribution rules of different syndromes of insomnia were analyzed. The internal relationship and correlation between the two syndromes were explored through the comparative analysis of physiological indicators such as Heart Rate Variability(HRV)between the different syndromes.Result1.In a survey of 124 undergraduate insomnia patients, the age distribution is in the range of 18?33 years old,the average age of the disease is x ±s=21.66 ± 3.22 years old,the prevalence of 18?21 years old is the highest, accounting for 64.5%.The problem of staying up late is common among college students, with 96.8%of them going to bed after 11 pm,and 56.5%of them going to bed after 0 am or even later.2.1n the constitution of TCM syndrome types of college students with insomnia, the syndrome of liver stagnation and spleen deficiency accounts for 37.1%,the syndrome of deficiency of both heart and spleen accounts for 29.0%,the syndrome of deficiency of heart and gallbladder qi accounts for 14.5%,the syndrome of yin deficiency and exuberant fire accounts for 10.5%,and the syndrome of liver stagnation and fire transformation accounts for 8.9%,with the syndrome of liver stagnation and spleen deficiency accounting for the most. There was no statistical difference among the insomnia syndromes in age structure and gender proportion.3.On factor 8 of SRSS(multiple dreams or being frequently awakened by nightmare),P<0.05 shows a statistically significant difference, suggesting that factor 8 has guiding significance in TCM syndrome differentiation and typing.4.According to one-way ANOVA,the differences between TP and ULF in different TCM syndrome types were statistically significant(P<0.05).5.Compared with the syndrome of exuberant fire due to yin deficiency, the syndromes of deficiency of both heart and spleen have the following significant differences(P<0.05):SDNN,SDNNI,RMSSD,PNN50,TP,LF,VLF,ULF,HR lbl,HR min,RR baseline,AHI,and the number of apnea. It indicated that SDNNs,SDNNI,RMSSD,PNN50,TP,LF,VLF,ULF,HR lbl,HR min,RR baseline,AHI,and the number of apnea episodes had guiding significance in TCM syndrome differentiation of insomnia.6.The differences of AHI between liver stagnation and spleen deficiency and yin deficiency with exuberant fire, and between liver stagnation and fire transformation type and yin deficiency with exuberant fire are all statistically significant(P<0.05),suggesting that AHI has guiding significance in TCM syndrome differentiation of insomnia.7.Compared with the syndrome of yin deficiency and exuberant fire, the TP and VLF of the syndrome of heart and gallbladder qi deficiency are both P<0.05,with statistically significant difference, suggesting that TP and VLF have guiding significance in TCM syndrome differentiation of insomnia.8.The ULF values of liver stagnation and spleen deficiency syndrome and liver stagnation and fire transformation syndrome, heart and spleen deficiency syndrome and liver stagnation and fire transformation syndrome, and liver stagnation and fire transformation syndrome and heart and gallbladder qi deficiency syndrome were all P<0.05,with statistically significant difference, indicating that ULF had guiding significance in TCM syndrome differentiation of insomnia.9.Pearson correlation analysis was performed between TCM syndrome types and physiological indicators, and P<0.05 was obtained, indicating a significant difference in statistics. The results showed that there was a correlation between TCM syndrome types and physiological indicators groups.Conclusion1.The most common insomnia syndromes in college students are liver stagnation and spleen deficiency, and the majority of the syndromes are deficiency-excess syndrome as well as deficiency syndrome.2.HRV and other physiological indicators have a certain correlation with insomnia syndrome.3.The correlation between TCM syndrome differentiation of insomnia and HRV and other physiological indicators needs to be further explored.
