| Objective:To explore the clinical efficacy of Shunqi Huatan Decoction in treating carotid atherosclerosis with stagnation of qi and phlegm,to evaluate the safety of drug treatment,and to provide clinical evidence for the treatment of carotid atherosclerosis with traditional Chinese medicine.Methods:From September 1,2020 to January 31,2021,26 patients diagnosed as Qi stagnation and phlegm coagulation carotid atherosclerosis in the encephalopathy department of Dongzhimen Hospital,Beijing University of Chinese Medicine were selected,and they were divided into 2 groups by a random number table,13 cases in each group.All patients should not use statins and other lipid-lowering drugs or other treatments that interfere with blood lipids one month before and during treatment,and suspend other traditional Chinese medicines that have the effect of invigorating qi,activating blood and reducing phlegm.At the same time of routine treatment of basic diseases(to ensure the balance of medication between groups),the experimental group(Shunqi Huatan Decoction group):Shunqi Huatan Decoction granules;control group(placebo group):Placebo granules,The two groups are taken 1 bag/time,morning and evening after dinner,the total course of treatment is 12 weeks.Both groups of patients were examined for blood lipids and carotid artery ultrasound before and after enrollment.The changes in the average intima-media thickness,plaque thickness,maximum plaque area,and lumen stenosis rate of the bilateral carotid arteries were observed before and after treatment in the two groups.,As well as changes in blood lipid indicators,changes in TCM syndrome scores,TCM and Western medicine clinical efficacy scores and safety indicators,etc.Statistics and analysis were performed by statistical methods,and the effectiveness and safety of Shunqi Huatan Decoction in the treatment of carotid atherosclerosis with stagnation of qi and phlegm were evaluated.Results:?Arterial color Doppler ultrasound results:After treatment,the average intima-media thickness and lumen stenosis rate of the Shunqi Huatan Decoction group were reduced compared with before,and the curative effect was significantly better than that of the control group.The difference was statistically significant(P<0.05)There was no significant change in the plaque thickness and the largest plaque area before and after treatment in the two groups(P>0.05).?Blood lipid examination results:By comparing the difference in blood lipid index between the two groups of patients before and after medication,it was found that the Shunqi Huatan Decoction group was significantly better than the control group in controlling triglyceride levels,and the difference was statistically significant(P<0.05).There was no significant difference in other blood lipid indexes before and after treatment(P>0.05).Evaluation of TCM syndromes:Comparing the TCM syndrome score scales of the two groups of patients before and after treatment,the TCM syndrome scores of patients in the Shunqi Huatan Decoction group after treatment were significantly improved compared to before treatment,and the efficacy was significantly better than that of the control group.It is statistically significant(P<0.05).? Evaluation of clinical efficacy of traditional Chinese and Western medicine:The clinical efficacy of TCM Shunqi Huatan Decoction group is significantly better than the placebo group,and after treatment,the total effective and significant efficiency of the medical effect of Shunqi Huatan Decoction group is significantly higher than that of the control group.The difference is significant.Statistical significance(P<0.05);there was no significant difference in the efficacy of western medicine and the total effective rate of western medicine between the two groups of patients(P>0.05)?There were no obvious adverse events and adverse reactions in the two groups of patients during the treatment.Conclusions:Shunqi Huatan Decoction can effectively reduce the average carotid intima-media thickness and lumen stenosis rate in patients with carotid atherosclerosis due to stagnation of qi and phlegm,but it has no obvious effect on reducing the formed plaque;Tan Decoction can control the patient's blood lipid level to a certain extent,but the overall lipid-lowering effect is limited;it has outstanding efficacy in improving the patient's clinical symptoms,and no obvious adverse reactions occurred after 12 weeks of treatment.It is a traditional Chinese medicine for the prevention and treatment of carotid atherosclerosis.The application of hardening provides strong evidence. |
