This research is mainly divided into the following two parts:1 Cross-sectional clinical studyObjective:To analyze the differences in lipid metabolism of patients with psoriasis and different syndromes by studying the clinical data of 138 cases of psoriasis vulgaris and 80 cases of healthy people,so as to provide a basis for clinical seeking objective diagnosis and treatment indicators.Methods:A cross-sectional questionnaire survey was used to collect clinical data on basic data such as body mass index,medical history,family history,dietary preferences,four-diagnosis information,and laboratory test indicators related to lipid metabolism.Data is integrated.Results:1.This study finally included 218 patients,including 80 in the healthy group and 138 in the psoriasis group.The general information of the two groups including age and gender were not statistically significant(P>0.05),suggesting the two groups Comparable between.2.A total of 138 patients with psoriasis were included in the experimental group,of which about half of the patients had a disease course ranging from 1 to 10 years;about 1/3 of the patients had a family history of psoriasis and a history of hyperlipidemia,combined with hyperlipidemia Patients with metabolic diseases such as chronic disease and fatty liver account for 12%of the total population with psoriasis;about 30%have bad habits(such as smoking,drinking),and those with acrimony and fatty liver account for about half of those with dietary preferences;statistically Thin veins are the most common in patients with psoriasis,and the tongue is thick and greasy.And the BMI of the psoriasis group was statistically significant compared with that of the healthy group(P<0.05).3.In terms of laboratoy test indicators,the psoriasis group compared with the healthy group TAG,HDL-C,HDL-C,VLDL,ApoB,LP(a),TC/HDL-C,AIP,ApoB/ApoA1 are statistically different(P<0.05),in which TAG,HDL-C,VLDL,ApoB,LP(a),TC/HDL-C,AIP,ApoB/ApoAl showed an increasing trend,and HDL-C showed a decreasing trend..Incorporating the above significantly different indicators into the binary logistics regression analysis,it is found that psoriasis is mainly associated with MBI.The larger the BMI index,the higher the risk of disease.However,there was no significant difference in TC,LDL-C,ApoA1 and other indicators between blood-heat syndrome and blood dryness syndrome in psoriasis(P>0.05).4.Conclusion:Based on the cross-sectional analysis of 138 patients with psoriasis,combined with the patient's history of hyperlipidemia,combined metabolic disease,poor eating habits,high BMI index,and tongue and pulse performance,it can be preliminarily inferred that the patient may have lipid metabolism The disorder is further verified by laboratory indicators.The lipid metabolism of patients with psoriasis is abnormal.which provides certain support for analyzing the pathological mechanism of psoriasis from the perspective of lipid metabolism.2 Basic experimental researchObjective:Based on UPLC-MS technology,to find different lipid metabolism markers of blood heat type and blood dryness type in patients with psoriasis vulgaris,and to clarify the possible mechanism of different syndromes of psoriasis.Methods:High-performance liquid chromatography-mass spectrometry(UPLC-MS)was used to detect the metabolites of peripheral serum from patients with psoriasis,and principal component analysis(PCA),partial least squares discriminant analysis(PLS-DA)and orthogonal partial least squares were used for the detection results.Analysis(OPLS-DA)modeling to explore changes in the overall lipid metabolite profile of psoriasis vulgaris serum samples.Results:1.There are 25 different compounds between the psoriasis group and the healthy group,including 6 phosphatidylcholine(PC)differentially expressed substances,PC 32:1,PC 32:2,PC 36:1,PC 38:2-1,PC 40:4-2,PC 40:5-1.There are 4 differentially expressed substances of sphingomyelin(SM),namely SM 36:1-2,SM 41:1,SM 42:1,and SM 42:2.There are 12 differentially expressed substances of triglyceride(TAG),namely TAG 480,TAG 481,TAG 500,TAG 501,TAG 502,TAG 503,TAG 522,TAG 523,TAG 524,TAG 525,TAG 541,TAG 543.There are 3 differentially expressed substances of Fatty acid(FA),namely FA 16:0,FA 18:1,and FA 18:2.The above substances are all highly expressed.2.There are 23 compounds with significant differences between the healthy group and the blood heat group,among which 6 are PC-type differentially expressed substances,PC 32:0,PC 32:1,PC 38:2-1,PC 40:4-2,PC 40:5-1 high expression;PC 33:3 low expression.There are 2 SM differentially expressed substances,of which SM 33:1 is lowly expressed and SM 36:1-2 is highly expressed.12 differentially expressed TAG substances,namely TAG 480,TAG 481,TAG 500,TAG 501,TAG 502,TAG 503,TAG 522,TAG 523,TAG 524,TAG 525,TAG 541,TAG 543,all of which are highly expressed.There are 3 FA differentially expressed substances,namely FA 16:0,FA 18:1,and FA 18:2,all of which are highly expressed.3.There are 20 compounds with significant differences between the healthy group and the blood dryness group,of which there are 2 FA compounds,FA 16:0 and FA 18:2,and 5 PC compounds,PC 32:0 and PC 32:1.PC 38:2-1,PC 40:4-2,PC 40:5-1;1 type SM,which is SM 36:1-2;12 types TAG,which are TAG 480,TAG 481,TAG 500,TAG 501,TAG 502,TAG 503,TAG 522,TAG 523,TAG 524,TAG 525,TAG 541,TAG 543;the above substances are all highly expressed.4.Six different compounds were identified in blood heat group and blood dryness group,namely LPE 20:1,LPC 19:0,PE 34:3-2,PC 42:11,SM 31:1,FA 15:0,the above substances are all highly expressed.Conclusion:Based on UPLC-MS analysis of serum lipid metabolism,it can be seen that there are differences in lipid metabolism expression between healthy group and psoriasis group,healthy group and various syndrome types,blood heat group and blood dryness group,and healthy group and psoriasis The expression of FA,PC,SM,and TAG in the blood heat group is significantly different;there are differences in the expression of LPE,LPC,PE,PC,SM,and FA in the blood heat group and the blood dryness group.The difference in metabolites is beneficial to the silver.The in-depth study of the differential markers of different syndromes of psoriasis,combined with technical methods such as genomics and transcriptomics,will further clarify the possible syndrome mechanisms of different syndromes of psoriasis.
