Research backgroundThe study of syndrome standardization is the only way to the modernization of traditional Chinese medicine.The lack of objective and uniform diagnostic criteria for syndromes is the bottleneck of the development of traditional Chinese medicine.Chronic prostatitis(CP)is a common disease of the genitourinary system,with a high incidence rate.Environment,diet,and lifestyle are closely related to CP,including smoking,drinking,sitting for long periods of time,frequent sexual intercourse,and delayed ejaculation,etc.In clinical practice,the most common combined syndrome of chronic prostatitis is damp-heat stagnation syndrome,which is a compound syndrome of damp-heat infusion syndrome and qi stagnation and blood stasis syndrome,rather than a simple addition of the two.Through literature search,it was found that the existing diagnostic criteria for CP damp-heat stasis syndrome are not uniform,and each item lacks clear terminology explanation and weight assignment,which restricts the clinical diagnosis and scientific research development of CP damp-heat stasis syndrome.This study used literature searches,expert questionnaire survey and principal component analysis to clarify the diagnosis entry,sort out the logical relationship between the entries,and assign weights to the entries at the same time to formulate unified diagnostic criteria for the chronic prostatitis with damp-heat stasis syndrome.Research methodsIn the early stage,the diagnostic criteria for damp-heat stasis syndrome were screened through literature search,the entries in each criterion were counted and the frequency of occurrence was calculated,and the entry pool was established,and the standard entry pool was established after discussion and screening by the expert group.we Invite members of the expert group to discuss on-site to determine the standard development method.Using the Delphi Method and questionnaires to collect expert opinions,analyze the data,and comprehensively use principal component analysis and other methods to determine the primary and secondary symptoms and assign weights to items.Research resultsBased on the item pool established by literature search,the Standards Committee held a meeting to discuss preliminary screening for relevant items and formulate questionnaires.Three rounds of national surveys of experts on sub-high level and above were carried out through a combination of on-site and remote methods to optimize items and assign weights,and develop a syndrome quantification table.The basis for the diagnosis of chronic prostatitis with damp-heat stasis syndrome was finally established.It consists of "layer of urination symptoms","pain symptom layer","tongue manifestation","pulses manifestation" and "other layers",a total of 27 items.Main symptoms:frequency,urgency,dysuria,distending pain or discomfort(perineum/waist/lower abdomen/lateral lower abdomen)Yellow greasy fur,ecchymosis on tongue,stringy rapid pulse,slippery rapid pulse,stringy slippery pulse.Secondary symptoms:dysuria,Post micturition dribble,Burning sensation in urine,turbidity after urine,difficulty urinating,distending pain or discomfort(testis testicle/public region/scrotum/anal region/inguinal region/hypogastrium),Wet scrotum,red tongue,yellow fur,slippery pulse.Diagnosis principles:(1)At least one main symptom of urination symptom and pain symptom,and one tongue manifestation.(2)According to the weighted total score,the degree of syndrome is graded:mild from 14 to 20 points,moderate from 21 to 35 points,and severe?36 points.Research conclusionThe diagnosis of damp-heat stasis syndrome is based on the establishment of modern medical disease diagnosis of chronic prostatitis.It includes diagnosis basis table and diagnosis principle.Table 1 The diagnosis basis of chronic prostatitis with damp-heat stasis syndrome(?)Diagnosis principles:(1)At least one main symptom of urination symptom and pain symptom,and one tongue manifestation.(2)According to the weighted total score,the degree of syndrome is graded:mild from 14 to 20 points,moderate from 21 to 35 points,and severe ? 36 points. |
