A Real-world Study On The Characteristics Of TCM Dominant Population In The Diagnosis And Treatment Of Advanced Colorectal Cancer By Professor Zheng Weiqin

Objective:Under the background of the era of big data,malignant tumor staging is closely related to the choice of treatment,the late modern medical treatment of colorectal cancer has entered the molecular target precision medical fields,such as whether the treatment of traditional Chinese medicine can also be implemented for the treatment of malignant tumors of the layered,individualization of advanced colorectal cancer,precision medical treatment,traditional Chinese medicine is a new direction for the prevention and control of malignant tumor.Clinically,different patients in the same disease may have completely different clinical responses to the traditional Chinese medicine treatment under the guidance of the same traditional Chinese medicine physician.In order to find suitable for treatment of traditional Chinese medicine of traditional Chinese medicine(TCM)aCRC advantages,give full play to the clinical value of traditional Chinese medicine,this topic based on the National Key Research and Development Program--Innovation Research on the Experience Inheritance of Famous Old Chinese Medicine Based on the Thought of "Doctrine Combination" and Multiple Integration Method(2018YFC1704100)and chongqing science and technology commission project--A real world study of Zheng Weiqin's TCM treatment of colorectal cancer based on HIS system and ct DNA analysis(cstc2019jxjl130013).To the fourth batch of national old Chinese medicine expert academic experience inheritance guidance teacher,chongqing Zheng Weiqin director of TCM doctor of traditional Chinese medicine treatment of advanced colorectal cancer(aCRC)in patients with clinical data for the gripper,through real world research method,according to the aCRC advantages of TCM population screening criteria,on Zheng Weiqin of diagnosis and treatment of traditional Chinese medicine aCRC filtered the advantage of the crowd.SPSS20.0 software was used to compare the difference between dominant population and non-dominant population to clarify the characteristics of TCM dominant population in Zheng Weiqin 's diagnosis and treatment of aCRC.To explore the characteristic prognostic factors in Zheng Weiqin 's TCM treatment of aCRC,and to provide a preliminary basis for further exploration of the possible mechanism of action of TCM treatment of a RCR as well as individualized and precise treatment.Methods:These aCRC patients who received oral TCM treatment after Zheng Weiqin's diagnosis and treatment in the outpatient or inpatient department of Nanqiao Temple Department and Dao Men Department of Chongqing Hospital of Traditional Chinese Medicine ftom April 1,2018 to December 30,2020 were selected to collect relevant data and TCM information.Patients were regularly followed up and their survival time was recorded.The differences among different populations were statistically analyzed.Univariate analysis and binary Logistic regression model were used to explore the characteristics of TCM dominant population in the diagnosis and treatment of aCRC by Professor Zheng Weiqin.Results:A total of 109 aCRC patients were included in this study.(1)Survival analysisOf the 109 patients,54 had died by the end of follow-up.The mean IV survival time of all enrolled patients was 36.35±22.95 months,and the median IV survival time was31 months.The mean IV survival time was 46.12±21.28 months and the median IV survival time was 41 months in the TCM dominant population group.Combination of traditional Chinese medicine and western medicine with anti-tumor therapy has certain curative effect and can prolong the survival time of aCRC patients.(2)Analysis on the characteristics of dominant population of TCM(1)The survival function curves of the dominant and non-dominant groups showed that the survival time of females was significantly higher than that of males.(2)There was a statistical difference in KPS between the two groups(P<0.05).The proportion of KPS?80 in the dominant group was higher(82.7% VS 58.8%),and the median survival time of those ?80 in the dominant group was higher(41.5months).(3)There was a statistical difference in surgical treatment between the two groups(P<0.05),and the proportion of patients receiving surgical treatment was higher in the dominant group(90.7% VS 73.5%).(4)There were statistical differences between the two groups in TCM syndromes of spleen and kidney deficiency and dampness toxin accumulation(P<0.05).Patients with TCM syndromes of spleen and kidney deficiency and dampness toxin accumulation in the dominant group were significantly higher than those in the non-dominant group(31cases VS 5cases).(5)two groups in the history of the intestinal polyps was statistically difference(P<0.05),the advantage of population groups with no history of intestinal polyps than higher than the dominant population groups(86.7% VS 79.4%),the dominant population groups in the history of no intestinal polyps patients significantly better than the average survival period has a history of intestinal polyp patients(46.82 ± 21.392 months VS29.33 ± 8.145months),Logistic model analysis shows there is no history of intestinal polyp patients closer to Zheng Weiqin aCRC TCM advantages of diagnosis and treatment of old people.(6)Those two groups has statistical difference in the tumor differentiation degree(P<0.05),the dominant population groups in poorly differentiated tumor significantly less than the advantage of population groups(4.0% VS 29.4%),Logistic model analysis shows that the tumor differentiation degree of aCRC independent prognostic factors for patients,the higher the degree of tumor differentiation of patients closer to Zheng Weiqin aCRC TCM advantages of diagnosis and treatment of old people.(7)There were significant differences between the two groups in the intervention period of TCM(P<0.05).Logistic model analysis showed that the intervention period of TCM was an independent prognostic factor affecting aCRC.Compared with traditional Chinese medicine intervention after the end of Western medicine anti-tumor treatment,starting TCM combined with Western medicine treatment after diagnosis can benefit more.Conclusion:Based on the 109 patients with advanced colorectal cancer included in this study,the study results indicated that the traditional Chinese medicine dominant population in the diagnosis and treatment of aCRC by Professor Zheng Weiqin might have the following characteristics:no history of intestinal polyps,differentiation in high school,earlier intervention period of traditional Chinese medicine,and the intervention time of traditional Chinese medicine >6 months.
