Objective:In this study,the effects of Taohong Siwu Decoction on platelet count,fibrinogen and D-2 polymer of traumatic spleen rupture patients undergoing splenectomy were observed to explore the therapeutic effect of Taohong Siwu Decoction on platelet hypertrophy after splenectomy and prevention of portal vein thrombosis.It provides a new idea and basis for traditional Chinese medicine to treat thrombocytosis after splenectomy and to prevent portal vein thrombosis.And optimizing splenectomy for the prevention of portal vein thrombosis.Methods:The subjects were 50 patients diagnosed with traumatic rupture of the spleen and underwent splenectomy in the Department of Hepatobiliary,Pancreatic and Vascular Surgery,Changsha Hospital of Traditional Chinese Medicine from February 2020 to February 2021.They were randomly divided into two groups:the experimental group(Taohongsiwu decoction+low molecular weight heparin sodium)and the control group(low molecular weight heparin sodium)with 25 patients each.After the diagnosis was confirmed on admission,the same group of doctors performed splenectomy under general anesthesia.Patients in both groups were routinely given symptomatic treatment such as anti-infection and fluid replacement after surgery.The number of platelets in patients was monitored.If the number of platelets was greater than 400,000,the control group was given 5000U subcutaneous injection of low molecular weight heparin sodium,and the experimental group was additionally given Taohong Siwu Decoction granules for oral treatment on the basis of the control group,and continued to take them for 1 week.Fasting blood routine,blood coagulation routine and D-2 polymer indexes were recorded in the morning of preoperative and postoperative days 1,3,7 and 10.The indexes such as hemoglobin,platelet count,fibrinogen content and D-2 polymer were selected and analyzed,and SPSS26.0 software was used for statistical data analysis.Results:(1)In the experimental group,1 patient died and Unable to collect data,thus the experiment was stopped.Therefore,25 patients in the control group and 24 patients in the experimental group were actually included,a total of 49 patients.(2)There were no significant differences between the two groups in gender,age,preoperative hemoglobin,platelet,D-2 polymer,fibrinogen,injury grade,operation time,total blood transfusion volume and other data(P>0.05).(3)Postoperative comparison between the two groups showed that there were no significant differences in hemoglobin,platelet,D-2 polymer and fibrinogen on the first and third day after surgery.The difference in hemoglobin at day 5 and 7 was statistically significant(P<0.05),and there was no significant difference on the 10th,14th and 20th day(p>0.05).Platelet,D-2 polymer and fibrinogen in the experimental group were better than those in the control group at the 5th,7th,10th,14th and 20th days,and the difference was statistically significant(P<0.05).Conclusion:Internal administration of Taohong SiWu Decoction combined with western medicine anticoagulant therapy has a significant effect on the treatment of platelet hyperplasia after splenic rupture and the prevention of postoperative portal vein thrombosis,which can well control the range of platelet hyperplasia in patients.It is beneficial to reduce the risk of postoperative portal vein thrombosis.Compared with conventional western medicine basic anticoagulant therapy is more conducive to obtain better curative effect.At the same time,no obvious adverse reactions were observed in this experiment.It has good safety and is worthy of clinical promotion.
