Efficacy Analysis Of Taohong Siwu Decoction Combined With Erchen Decoction In Preventing Lower Extremity Deep Vein Thrombosis After Total Knee Arthroplasty In Patients With BMI≥24kg/m~2

Objective: Objective to analyze the curative effect of Taohong Siwu Decoction and Erchen decoction in preventing lower extremity deep venous thrombosis in patients with BMI ≥ 24kg/ m2 after total knee arthroplasty,and to provide evidence-based medical evidence for clinical Chinese medicine in preventing lower extremity deep venous thrombosis in patients with BMI≥ 24 kg / m2 after total knee arthroplasty.Methods: Sixty Patients with BMI≥24kg∕m2for primary total knee arthroplasty were randomly divided into two groups,and 30 cases were treated with traditional Chinese medicine group,for routine Postoperative(subcutaneous injection of natroheparin calcium)to prevent DVT(Deep venous).Thrombosis(DVT)and Chinese traditional medicine decoction were added.In the conventional western medicine group,30 cases were treated with low molecular weight heparin calcium.The changes of D-dimer(DDi),FIBrinogen(FIB),Activated partial thromboplastin time(APTT),Prothrombin time(PT)and the incidence of postoperative DVT in 2 groups were observed before surgery,1,3 and 14 days after surgery.Results: A total of 56 patients completed the study plan and completed the study index data:observation group(n = 28)and experimental group(n = 28).1.There was no statistical significance in age,body mass index and underlying diseases between the two groups(P > 0.05).2.Laboratory indicators: DDI,APTT,PT,FIB: there was no statistical significance in the difference between the two groups before surgery and the first day after surgery(P > 0.05);The values of laboratory indexes in the group treated with Chinese medicine were lower than those in the group treated with western medicine alone on the 3rd and 14 th day after surgery,with statistical significance(P < 0.05).3.Incidences of DVT: no patients in the observation group had deep vein thrombosis,while 2patients in the western medicine group had calf vein thrombosis on the affected side,with no statistical significance(P > 0.05).4.Evaluation of TCM syndrome efficacy score: The TCM syndrome score of the group receiving TCM plus medicine after drug treatment was lower than that of the group receiving western medicine alone,with statistical significance(P < 0.05).5.Safety Assessment: During the treatment,no drug allergic reaction,abnormal liver and kidney function,bleeding of vital organs and dangerous bleeding were found in the two groups,no pulmonary embolism and related symptoms and signs were found,and no bleeding of gingiva or digestive tract was found.Conclusion: On the 3rd and 14 th day after total knee arthroplasty in patients with BMI ≥24kg / m2,oral administration of Taohong Siwu Decoction and Erchen decoction can improve the indexes of DDi,APTT,PTand Fib in patients’ serum,improve the hypercoagulable state of blood after operation,improve the TCM syndrome,prevent the formation of DVT,and accelerate the recovery.
