Objective:To detect the expression levels of peptidylarginine deiminase 4(PADI4)and human leukocyte antigen allele,DRB1*11(HLA-DRB1*11)in peripheral blood leukocytes of patients with cerebral infarction and control population,analyze the correlation between two genes and acute phase of cerebral infarction,and their influence on the prognosis of cerebral infarction.Methods:The case group selected 80 patients with acute cerebral infarction who were hospitalized from March 2020 to October 2020 and met the enrollment criteria.The control group selected 80 patients with non-cerebral infarction who were hospitalized during the same period.The clinical data of the two groups of patients were collected,and the expression levels of PAD 14 and HLA-DRB1*11 genes in the peripheral blood leukocytes of the two groups were detected by real-time fluorescent quantitative PCR(RT-qPCR).We compared the expression differences between the two groups in the two groups,analyzed the relationship between gene expression levels and prognosis,and evaluated the diagnostic efficacy of the prognosis of cerebral infarction by drawing a receiver operating characteristic curve(ROC).Results:1.The expression of PADI4 gene in the case group was higher than that in the control group,and the difference was statistically s i gni fi c ant(P<0.05).Compared with the good prognosis group,PADI4 gene expression increased in the poor prognosis group,and the difference was statistically significant(P<0.05).Logistics regression analysis showed that high expression of PADI4 gene was associated with poor prognosis of cerebral infarction(OR=3.389,95%CI:1.29 1-8.902).ROC curve result showed that high expression of PADI4 gene had moderate prognosis efficacy for poor prognosis of cerebral infarction(AUC=0.726).Correlation analysis results showed that PAD I4 gene expression level was positively correlated with NIHSS scores on day 3 and day 7.2.There was no significant difference in HLA-DRB1*11 gene expression between the case group and the control group(P>0.05).There was no significant difference in HLA-DRB1*11 gene expression between patients with good prognosis and patients with poor prognosis(P<0.05).Logistic regression analysis showed that there was no significant correlation between HLA-DRB1*11 gene expression and prognosis of cerebral infarction(OR=0.976,95%CI:0.3 73-2.554).Conclusion:1.The expression of PADI4 gene increased in the acute phase of cerebral infarction.High expression of PAD I4 gene was associated with poor prognosis of cerebral infarction.2.The expression level of HLA-DRB1*11 gene did not increase significantly in the acute phase of cerebral infarction,and it was not significantly related to the prognosis of cerebral infarction.
