| Objective: To observe and evaluate the clinical effect of Yangchao Zengmo Decoction on the ovarian function and endometrial receptivity of IVF-ET elderly infertile women.The research provides clinics for Yangchao Zengmo Decoction to improve the clinical pregnancy rate of IVF-ET elderly infertile women and the clinical evidence for the multi-system,multi-target,and multi-step regulation of reproductive function in Chinese medicine.Methods: Sixty patients who met the diagnostic criteria and TCM criteria for kidney deficiency and blood stasis differentiation were numbered according to the order of entry time,and were divided into 30 cases for control group and 30 cases for treatment group according to the random number table method.The control group was treated with estradiol valerate tablets(Bujiale,Bayer Pharmaceutical Co.,Ltd.,Germany)+ Dydrogesterone tablets(Duffton,Solvay Pharmaceutical Co.,Ltd.,Netherlands)for sequential treatment for 3 consecutive cycles.The treatment group patients was added with Yangchao Zengmo Decoction on the third day of clean menstruation on the basic treatment for 18 days,and the drug was stopped during menstruation.Repeated 3 consecutive cycles.The levels of endocrine hormones,the number of antral follicles,and the scores of TCM syndromes of the two groups were observed before and after treatment.Both groups underwent IVF-ET(antagonist regimen)in the 4th cycle and underwent fresh cycle transplantation.The differences in endometrial thickness,endometrial type,endometrial blood flow classification on the day of HCG injection were compared between the two groups,and the differences between the two groups in the number of oocytes retrieval,cycle cancellation rate,embryo implantation rate,clinical pregnancy rate and and early abortion rate were compared.Results: Comparing the scores of TCM clinical syndromes,the treatment group was significantly improved compared with the control group(P=0.000).Compared with the total effective rate of clinical curative effect of traditional Chinese medicine,the treatment group is better than the control group(P<0.05).In improving the serum index,both groups can reduce the levels of FSH and LH(P<0.05),and the therapeutic effect of the treatment group was significantly better than that of the control group(P=0.000).In the improvement of B-ultrasound index,the comparison between the two groups and the number of antral eggs showed that the treatment group was better than the control group(P<0.05).Comparing the endometrial thickness and endometrial type on the day of HCG injection,the treatment group was significantly better than the control group(P=0.000).Compared with Applebaum classification of endometrial blood flow on the day of HCG injection,the treatment group was better than the control group(P<0.05).Comparing the number of oocytes retrieval of the two patients,the number of oocytes retrieval in the treatment group(4.93±1.28)was significantly better than the control group(2.03±0.85)(P=0.000).Comparing the cycle cancellation rate of the two patients,the cycle cancellation rate of the treatment group(6.7%)was significantly lower than that of the control group(36.7%)(P=0.000).Comparing the early abortion rate between the two groups,the early abortion rate in the treatment group(20.0%)was significantly lower than the early abortion rate in the control group(54.4%)(P=0.000).Comparing the embryo implantation rate between the two groups,the treatment group(81.1%)was better than the control group(52.2%)(P<0.05).Comparing the clinical pregnancy rate of the two groups,the clinical pregnancy rate of the treatment group(89.3%)was significantly better than that of the control group(57.9%)(P=0.000).Conclusion: 1.Yangchao Zengmo Decoction can significantly improve the symptoms of kidney deficiency and blood stasis in patients with TCM;2.Yangchao Zengmo Decoction can reduce serum FSH and LH levels,increase the number of antral follicles,thereby promote ovulation and increase the number of eggs obtained;3.Yangchao Zengmo Decoction can increase the thickness of the endometrium,improve the blood perfusion of the endometrium,and increase the receptivity of the endometrium,thereby reducing the cycle cancellation rate and early abortion rate,and increasing its embryo implantation rate and clinical pregnancy rate. |
