Study On The Quality Of Home Care Service In Designated Nursing Stations Of Long-term Care Insurance In Shanghai

In recent 40 years,the birth rate in Shanghai has been declining year by year,leading to the social population structure of the elderly and fewer children in Shanghai.The health care of the elderly has become an important subject of medical and civil administration.The Twelfth Five-Year Plan for the Development of Civil Affairs in Shanghai has determined the pension pattern of 9073(90% of the elderly rely on the community for home care,7% of the elderly in the community,and 3% of the elderly in institutions).In 2015,Shanghai initially formed a home-based old-age care pattern,taking the lead in carrying out the nursing station service mode to provide home-based care services for the elderly.In 2016,the Ministry of Human Resources and Social Security issued the Guiding Opinions of the General Office of the Ministry of Human Resources and Social Security on the Implementation of the Pilot Long-Term Care Insurance System(No.80 [2016] issued by the Ministry of Human Resources and Social Security).Shanghai was one of the first pilot cities,and the long-term care service was developed.In the long-term home care service,the nursing station under the long-term care insurance has become the main supply of the elderly to enjoy the home care service.By the end of 2020,as the first pilot areas in Shanghai,Xuhui District,Putuo District and Jinshan District have built a number of nursing stations,which play a role as a benchmark for development.This paper selects Xuhui District as the research site to obtain more abundant first-hand information.This paper takes the "healthy China 2030" planning outline "the development of the elderly long-term care services that occupy the home" as the guidance,to the Shanghai trial of long-term care insurance method "(Shanghai Fu Fa [2017] no.97)and the measures for the administration of the nursing station in Shanghai(Shanghai health medical administration [2016] no.46)as the theoretical guidance,based on customer perceived service quality theory and SERVQUAL model of service quality,long-term care services can be divided into basic life care and clinical medical care services,The index system for evaluating the quality of home care service supply in designated nursing stations of Shanghai long-term care insurance was constructed from five dimensions of tangibility,reliability,assurance and caring.The questionnaire and interview outline were designed.After two rounds of preliminary investigation,the final version of the questionnaire was determined.After that,a total of 226 questionnaires were sent out and 204 valid questionnaires were received.Finally,the long-term home care service quality of Shanghai nursing station and its influencing factors were analyzed by ANOVA and orderly multi-classification logistic regression method,and relevant conclusions were drawn and targeted measures were put forward.Found related to this article is in nursing station to provide long-term home care service quality evaluation process,the results showed that only two indicators of service quality is positive,namely the physical indicators of basic life care and common clinical care responsiveness indicators,the rest of the eight indicators of service quality is negative,that nursing station to provide home care service quality satisfaction with the old man.There is still a large gap between Logistic regression analysis showed that gender,age,the situation of the people living with them and whether they had received other services all affected the elderly’s evaluation of the long-term home care service quality provided by the nursing station.According to the research findings,in the process of providing long-term home care services,there are some problems,such as unclear service rules of nursing stations,low service quality and skill level of nursing staff,and low sense of security for the elderly.Therefore,relevant suggestions are put forward to improve the system and norms of nursing stations,increase the team construction of home care service personnel,improve the quality of care personnel and promote the communication of multiple subjects,so as to jointly promote the highquality development of home care service of long-term care insurance designated nursing stations.
