BackgroundBased on psoriasis vulgaris patients’lack of disease cognition and disease self-management ability,low quality of life,and more prone to anxiety and depression than normal people,this study focuses on the group of outpatient psoriasis vulgaris patients.Drawing on experience in the application of empowerment theory at home and abroad,the empowerment education model was applied to outpatient psoriasis vulgaris patients to evaluate its impact on patients’disease cognition,medication compliance,self-health management ability,quality of life and psychological status,to provide new ideas for future dermatological care work.Objective1.Understand the needs of health education services for outpatient psoriasis vulgaris patients,and build a health education program for psoriasis vulgaris patients based on empowerment theory.2.To analyze the application effect of this health education program on outpatient psoriasis vulgaris patients,and provide reference for the treatment and nursing of outpatient psoriasis patients.MethodsThe research content is divided into three stages.The first is the status quo investigation:To investigate the outpatient visits of psoriasis vulgaris patients in the researcher’s hospital in recent years;to understand and analyze the health education needs of outpatient psoriasis vulgaris patients.The second is the construction of the health education intervention program based on the empowerment theory:According to the results of the investigation of the health education needs of patients with psoriasis vulgis,the preliminary program of the empowerment education was formulated.Through the expert group meeting and preliminary test,the suggestions of the expert group and the opinions of patients were summarized to form the empowerment education intervention program.Third,the implementation and effect analysis of health education intervention program based on empowerment theory:Using simple random sampling method,choose from October 9,2020 to October 9,2021,the researchers on the hospital dermatology clinics,98 conforms to the inclusion criteria of patients with psoriasis vulgaris,according to the order of time group,49 cases as control group before and after 49 cases as intervention group,control group routine health education,The intervention group received health education based on empowerment theory.Before the intervention and 6 months after the intervention,the psoriasis health knowledge and health behavior questionnaire,the patient medication compliance questionnaire,the chronic disease patient self-health management ability assessment scale,the skin disease patient quality of life scale,and the hospital anxiety and depression scale were used.The patients were evaluated,and statistical methods such as standardized effect index,t test,and chi-square test were used to analyze and compare the differences between the two groups of patients in terms of disease cognition,medication compliance,self-health management ability,quality of life,and psychological status.,to evaluate the actual effects of enabling educational interventions.Results1.Status investigation and analysis:A total of 210 outpatients with psoriasis vulgaris were investigated,with an average age of(34.87±13.95)years;The demand for health education among patients with psoriasis vulgaris in outpatient department was as following:89.05%thought it was necessary,7.62%did not care about it,and 3.33%did not need it.Patients’demands for health education contents included dietary guidance(94.76%),lifestyle(90.95%),medication guidance(97.14%),skin care(98.10%),family support(85.24%),emotional and psychological support(76.67%).The most desired guidance institutions were public hospitals(83.33%),community medical institutions(9.52%),psoriasis related websites(5.71%),and private clinics(1.43%).2.Effect analysis of health education intervention based on empowerment theory:Comparison of cognitive and behavioral levels between the two groups:after intervention,the standardized effect index of patients’knowledge was 2.28,and the standardized effect index of patients’behavior was 4.74,both higher than 1.17.The improvement of knowledge and behavior in the intervention group was significantly better than the control group.Comparison of medication adherence between the two groups:the intervention group accounted for 43.2%,36.4%,and 20.5%of high adherence,moderate adherence,and low adherence,respectively,while the control group accounted for high adherence,moderate adherence,and low adherence.The rates were 19.1%,21.3%,and 59.6%,respectively;the medication compliance of the intervention group was significantly higher than that of the control group(x~2=14.630,P=0.001).Comparison of self-management ability between the two groups:In the intervention group,cognitive ability(4.06±0.24),psychological status(4.09±0.47),behavioral lifestyle(4.01±0.39),social environment(4.07±0.58),treatment compliance(4.00±0.28),total score(4.05±0.17),In the control group,cognitive ability(3.02±0.29),psychological status(3.29±0.59),behavioral lifestyle(3.24±0.46),social environment(3.05±0.68),treatment compliance(3.02±0.22),total score(3.12±0.23),The total score and scores of each dimension in the intervention group were significantly better than those in the control group,with statistical significance(P<0.05).Comparison of quality of life between the two groups:DLQI score of the intervention group was(4.18±2.51)and that of the control group was(7.11±2.92).DLQI score of the intervention group was significantly lower than that of the control group,the difference was statistically significant(P<0.05),and the post-intervention score of the intervention group was significantly lower than that before the intervention,the difference was statistically significant(P<0.05).Comparison of psychological state between the two groups of patients:in anxiety,the score of intervention group was(5.82±2.83),the control group was(8.98±2.54);The score of depression was(5.77±2.72)in the intervention group and(8.96±2.35)in the control group.The score of anxiety and depression in the intervention group was significantly lower than that in the control group,with statistical significance(P<0.05),and the score after intervention in the intervention group was significantly lower than that before intervention(P<0.05).ConclusionsHealth education based on empowerment theory not only empowers patients’self-management ability,but also provides patients with multi-level individualized needs.This educational program can significantly improve patients’cognitive behavior level and medication compliance,and enhance self-health Management ability,improve psychological status,and then significantly improve the quality of life.
