BackgroundAlcohol dependence,also known as alcohol addiction,is a recurrent chronic encephalopathy.According to the 2018 WHO global report on alcohol use and health,the prevalence of AD among people reached 2.6%over 15 years old in 2016,and caused about3 million deaths worldwide,accounting for about 5.6%of all disease deaths.Its formation is closely related to personality characteristics,social environment,psychological characteristics,genetic heredity and neural factors.Alcohol addicts often have aggressive impulsive behavior,personality disorder,cognitive disorder and decision-making disorder,and AD is one of the important risk factors for marital breakdown,physical illness,disability,and even death.By analyzing the aggressive impulsive behavior,personality status,decision-making ability and its influencing factors in patients with alcohol dependent,this experiment hopes to provide effective help for their rehabilitation and prevention.Objectives1.To analyze the aggressive impulsive behavior and its possible influencing factors in patients with alcohol dependent,.2.To analyze the personality traits in patients with alcohol dependent.3.To analyze the decision-making ability and its correlation with drinking history,starting drinking habits,aggressive impulsive behavior,personality traits,and its possible influencing factors in patients with alcohol dependent.Methods1.120 AD patients who met the inclusion criteria were collected,of which 118completed the evaluation,the control group was 45 normal healthy people.2.In one week,the AD patients and normal group volunteers were tested for AUDIT,MOAS,MMPI,GDT,IGT and other tests in 3 times.And the AUDIT was used to screen the risk and degree of drinking,the MOAS was used to assess impulsive aggression,the MMPI was used to assess personality traits that are abnormally psychological or psychiatrically abnormal,the GDT was used to evaluate risky decisions with exact probability and the IGT was used to test risky decisions with ambiguous probability.3.Using spss25.0 software for analysis,the counting data are analyzed by chi square test,and the measurement data are analyzed by()indicates that it conforms to the normal distribution.Independent sample t-test is used for the comparison between the two groups,and two sample repeated analysis of variance is used for the comparison between the data at each time node of Iowa game test.Pearson correlation analysis is used for the correlation comparison of data subject to normal distribution,and Spearman correlation analysis is used for data not subject to normal distribution.Binary logistic regression is used to analyze the influencing factors of violence,and multiple linear regression is used to analyze the influencing factors of decision-making function.Results1.General information.There was no significant difference in age,years of education,educational level and household registration between the experimental group and the normal group(t/χ~2=-0.721,-0.740,0.786,all P>0.05),There were significant differences in drinking history,marriage,age of starting drinking,past violence and audit score(t/χ~2=2.507,-4.132,57.018,13.519,all P<0.05).2.The result of the MOAS score.According to the cut-off value of MOAS scale 4,104(88.1%)patients in the experimental group had impulsive aggressive behavior,and 9(20.0%)of the subjects in the normal group had impulsive aggressive behavior.The patients in the experimental group were similar to those in the normal group.The difference was statistically significant(χ~2=71.118,P>0.05).Low education,unmarried,rural household registration,previous violent behavior,combined with other diseases,lower income level and smoking history are risk factors for impulsive aggressive behavior in alcohol addiction patients(B=0.287,0.521,0.319,0.449,-0.502,0.879,0.151,χ~2=4.463,14.127,7.286,0.475,8.041,8.528,6.381,all P>0.05).3.The results of the MMPI score scale.There were significant differences between the experimental group and the normal group in the scores of MMPI scale hypochondriasis,depression,hysteria,psychosis,paranoia,mental weakness,schizophrenia,hypomania,doubt scale,lying scale and fraud scale(t=4.785,5.169,18.393,14.656,5.971,4.303,6.766,3.583,2.089,4.465,-3.178,all P<0.05).4.The result of the GDT and IGT test.There were significant differences in the selection of one number,three numbers,four numbers of GDT,total assets and negative feedback utilization between AD patients in the experimental group and the normal group(t=9.955,-6.533,-5.268,-8.799,-13.548,all P<0.05);The results of multivariate analysis of variance showed that the main effect within the group was not significant(F=0.126,P=0.723),the main effect between the groups(F=1.033,P<0.05)and the interaction effect(F=51.849,P<0.05)were significant.There were significant differences in the selection of two steps after IGT between AD patients in the experimental group and normal group(t=-8.945,-6.732,all P<0.05),The results of multivariate analysis of variance showed that the main effect was significant within the group(F=22.045,P<0.05)and between the groups(F=44.214,P<0.05)and interaction effect(F=29.384,P<0.05).5.Influencing factors of the GDT risk options and the IGT Net Score.Multiple linear regression analysis showed that the main influencing factors of high GDT risk options were years of education(B=-0.063,β=-0.262,P<0.05),and time to start drinking(B=1.788,β=0.286,P<0.05),MOAS scale score(B=1.967,β=0.142,P<0.05),hypomania score(B=1.515,β=0.184,P<0.05),paranoia score(B=0.265,β=0.346,P<0.05)and mental weakness score(B=0.048,β=0.125,P<0.05),etc(R~2=0.272,F=15.023,P<0.05).The main influencing factors of low IGT net score were drinking history(B=-3.091,β=-0.190,P<0.05),time to start drinking(B=-1.120,β=-0.430,P<0.05),MOAS scale score(B=-0.945,β=-0.421,P<0.05),depression score(B=0.648,β=0.234,P<0.05),hypochondriasis score(B=-0.378,β=-0.185,P<0.05)and delusional score(B=-0.755,β=-0.336,P<0.05),etc.(R~2=0.788,F=68.922,P<0.05).Conclusions1.AD patients have a high proportion of impulsive aggressive behavior,accompanied by changes in personality traits such as mood and psychotic symptoms.2.AD patients have decision-making ability obstacles,and they tend to make risky decisions whether the risk is clear or vague. |
