Objective: To explore the relationship between preoperative blood lipid level and clinicopathological features and prognosis of breast cancer patients,and to evaluate whether preoperative blood lipid level can be used as a predictor of long-term prognosis of breast cancer patients.Methods: The levels of fasting serum total cholesterol(TC),triglycerides(TG),high density lipoprotein cholesterol(HDL-C)and low density lipoprotein cholesterol(LDL-C)in the morning and clinical pathological data of 341 breast cancer patients confirmed by pathology were collected retrospectively.Qualitative data using ?2test,rank data were tested by Kruskal-Wallis rank sum test,the Cox proportional risk regression model was used for univariate and multivariate analysis to test independent significant variables,and Kaplan-Meier analysis was used to analyze overall survival(OS)and disease-free survival(DFS)of breast cancer patients.Results:TC level was correlated with tumor stage(P=0.005)and lymph node stage(P=0.004),TG level was correlated with tumor stage(P<0.001)and tumor molecular typing(P=0.005).HDL-C levels correlated with tumor stage(P=0.002),LDL-C levels correlated with tumor stage(P=0.002)and lymph node stage(P<0.001).Cox proportional hazards regression model showed that low TG,high HDL-C and low LDL-C were beneficial to DFS.Patients with TC levels < 6.2mmol/L VS.patients with TC levels ≥6.2mmol/L,mean OS: 96.2 months VS.95.4 months(P = 0.556),mean DFS of 95.9 months in patients with TC levels < 6.2mmol/L and mean DFS of 94.0 months in patients with TC levels ≥6.2mmol/L(P=0.122).Patients with TG levels < 2.3mmol/L VS.patients with TG levels ≥2.3mmol/L,mean OS: 96.7 months VS.94.1 months(P =0.103),mean DFS 96.4 months in patients with TG levels < 2.3mmol/L and mean DFS92.1 months in patients with TG levels ≥2.3mmol/L(P=0.003).Patients with HDL-C level < 1.04mmol/L VS.patients with HDL-C level ≥1.04mmol/L,mean OS: 94.7months VS.97.6 months(P = 0.019),Mean DFS of 93.2 months in patients with HDL-C level < 1.04mmol/L and mean DFS of 97.6 months in patients with HDL-C water ≥1.04mmol/L(P=0.003).Patients with LDL-C <3.4mmol/L VS.patients with LDL-C ≥3.4mmol/L,mean OS: 96.8 months VS.94.1 months(P=0.026),mean DFS of 96.5 months in patients with LDL-C < 3.4mmol/L and mean DFS of 92.1 months in patients with LDL-C ≥3.4mmol/L(P=0.002).Conclusion: Preoperative blood lipid levels have a certain correlation with tumor stage,lymph node stage and molecular typing of breast cancer in patients with breast cancer.Preoperative TG level,HDL-C level and LDL-C level may be relevant predictive factors affecting DFS in breast cancer patients.
