Objective To explore the effect of pulmonary rehabilitation on respiratory function,walking ability,ADL and quality of life in convalescent stroke patients.Methods 40 stroke convalescent patients hospitalized in our hospital from April 1,2020 to March 31,2021 were selected.They were randomly divided into control group and observation group with 20 cases each.This study was reviewed and approved by the Ethics Committee of the Second Affiliated Hospital of Anhui Medical University[PJ-YX2018-038(F1)].During the period of hospitalization,the control group received routine rehabilitation treatment,such as comprehensive training of hemiplegic limbs,exercise therapy,balance function training,gait training,acupuncture,neuromuscular electrical stimulation,occupational therapy,muscle strength training,posture transfer,turning over training and good posture position placement.The observation group received pulmonary rehabilitation therapy on the basis of routine rehabilitation therapy,including deep breathing machine and external diaphragm pacemaker.Before discharge,therapists guided and taught home rehabilitation methods,including good posture,homework therapy,standing training,muscle strength training,etc.Respiratory function(FVC,MEP,MIP and FEV1),walking ability(6MWD,mean walking speed and maximum stride speed),daily living ability and stroke quality of life were evaluated 7 times before treatment,2 weeks after in-hospital rehabilitation and 2 weeks after home rehabilitation.2 weeks of in-hospital rehabilitation and 2 weeks of home rehabilitation were 1 course,and 3 courses of observation were 12 weeks in total.Results 1.Before treatment,two group respiratory function(FVC,MEP,MIP and FEV1),walking ability(6MWD,average stride speed and maximum stride speed),activities of daily living(Barthel index)and quality of life in convalescent stroke patients,there was no significant difference between groups(P>0.05).After each course of treatment,the results of all indicators in both groups were improved compared with before treatment,and the indicators in the observation group were improved more obviously than before treatment.2.After each course of treatment,there were statistically significant differences in FVC,MEP,MIP and FEV1 between 2 groups(P <0.05).There were statistical differences in all indicators of walking ability between the two groups(P < 0.05),and the observation group was better than the control group.After the first course of treatment,there were no significant differences in 6MWD,average step speed and maximum stride speed between groups(P > 0.05),while there were significant differences between groups after the second and third course of treatment(P < 0.05).After each course of treatment,the intra-group comparison of 2groups was statistically significant(P < 0.05),and the inter-group comparison of daily living ability was significantly different(P < 0.05).After the first and second course of treatment,there was no statistical difference in the quality of life of stroke between the groups(P > 0.05),but after the third course of treatment and the follow-up evaluation group,there was statistical difference(P < 0.05).With the increase of treatment course,the indicators of the observation group(quality of life at week 10)were significantly better than the control group.Conclusion Pulmonary rehabilitation therapy based on the conventional treatment regimen for stroke significantly improved the respiratory function and ADL from week2,walking ability from week 4,and quality of life after week 10 compared with conventional treatment.Pulmonary rehabilitation can improve the respiratory function of stroke patients,improve the walking ability,the ability of daily living activities and quality of life,promote the comprehensive functional recovery of stroke patients,return to family and society at an early date.
