Objective: To explore the correlation between histopathological features of pigmented nevus and age,location and dermoscopy.Methods: From June 2019 to December 2020,279 patients who were initially diagnosed with nevus were selected from the dermatology outpatient department of Suzhou Hospital Affiliated to Nanjing Medical University,including 279 cases of skin lesions.Dermatoscopy and pathological examination after surgical resection were performed to confirm the classification of all skin lesions.The age,gender,location,dermoscopic characteristics,histopathological classification and other characteristics of all the patients with nevus were counted and comprehensively analyzed.SPSS22.0was used for data processing.Results: 1.Basic information: A total of 279 patients were included in this study,including 279 skin lesions.Through pathological examination,231 cases were confirmed as pigmented nevus,and 48 cases were confirmed as seborrheic keratosis,melanocytoma,fibroma,etc.Among the 231 cases of nevus,54 cases were male and177 cases were female,with a male to female ratio of 1:3.3.The average age was35.71 years old(3 to 83 years old).Among them,there were 12 cases lesions in 0-14 years old,33 cases lesions in 15-29 years old,138 cases lesions in 30-44 years old,39 cases skin lesions in 45-59 years old and 9 lesions in >59 years old.There are 90 lesions on facial,27 lesions on trunk,18 lesions on limbs and 96 lesions on hands and feet.There was no significant difference between the distribution of acquired melanocytic nevus at different location and age(P>0.05).3.The distribution difference of different pathological types of nevi in different parts of the body was statistically significant(P=0.000).Intradermal nevi were mainly distributed in the head and face,followed by the hands and feet and the trunk,and the extremities.Junction nevus was mainly distributed in the hands and feet.Complex nevi were mainly distributed in the hands and feet.4.There were statistically significant differences in the distribution of pigmented nevi between different pathological types by homogeneous mode and parallel groove mode(P=0.0002,P=0.000).The most common nevus was intradermal,followed by junctional nevus.In parallel sulcus pattern,the most common nevus was junction nevus.5.There were statistically significant differences in the distribution of blue and white tentorium and vascular structure among different pathological types of pigmented nevi(P=0.000166,P =0.022),blue and white curtains were observed in 43 cases of intradermal nevi,7 cases of compound nevi,and 3 cases of borderline nevi.Vascular structures were found in 33 of the intradermal nevi,3 of the compound nevi,and 6of the borderline nevi.Conclusion: This study shows that there is a certain correlation between the pathological type and location of pigmented nevus as well as the dermoscopic manifestations.Most dermoscopic findings correspond to well-defined histomathological findings,and can be roughly inferred from dermoscopic images,and vice versa.If clinicians can realize the skin pigment nevus lens performance and the relationship between histopathology,so you can use the skin endoscopy pigmented nevus accurate assessment and management,especially for the blue and white curtain or vascular structures and other atypical skin mirror mode of melanin cell pathological changes,can use the regular monitoring of the development of skin lesions,skin lens reduce blind biopsy.
