| [Objective] to investigate the effect of different flushing interval time on continuous parenteral nutrition infusion in tumor patients through implantable venous port,and to reduce the tube plugging rate of tumor patients.[Methods] from October2019 to July 2020,127 cancer patients who received parenteral nutrition through implantable intravenous infusion port were selected as the research objects,and they were randomly divided into three groups,namely,the patients were flushed once every 2 hours,4 hours and 6 hours during the period of parenteral nutrition infusion through implantable intravenous port Occlusion:(1)unobstructed catheter refers to the withdrawal of blood and / or the maximum number of infusion drops ≥ 80 drops /min;(2)partial blockage of catheter refers to no blood return,no resistance or slight resistance of injection,and / or 0 drop / min < maximum infusion drop < 80 drops /min;(3)complete blockage of catheter refers to no blood return and no injection at all,and / or the maximum number of infusion drops = 0 drop / min The tube plugging rate of each group was counted,and the intra group difference and inter group difference of the fastest infusion drip number of the three groups were counted,and the correlation between the plugging rate and age,gender,platelet,D-2 polymer,plasma fibrinogen,diagnosis,use of chemotherapy drugs and targeted therapy were also counted.[results] 1.When comparing the plugging rate of the three groups,the plugging rate of the 2-hour flushing group was significantly lower than that of the6-hour flushing group(P = 0.03,and the difference was edge positive after FDR correction(P = 0.09).There was no significant difference between the two groups before and after FDR correction.2.There was a general difference in the number of the fastest infusion drops on the last day among the three groups(P = 3.81e-06).There was no significant difference between the 2-hour group and the 4-hour group(P >0.05).The 2-hour group was significantly higher than the 6-hour group(P = 5.68e-05).The 4-hour group was also significantly higher than the 6-hour group(P = 4.68e-05).3.According to the paired rank sum test,during the period of PN infusion by cvpas,the fastest drip number on the last day in the two groups was significantly higher than that in the first day,and the last day with an interval of 6 hours was significantly lower than that on the first day.4.From the multivariate logistic regression analysis,it can be concluded that the interval time of flushing tube,lung cancer and gastric cancer were significantly correlated with the blockage of parenteral nutrition(P <0.05),which were independent risk factors.[Conclusion] there was no significant difference between the two groups(P > 0.05),but the two-hour group consumed more manpower and material resources than the 4-hour group.Considering the above situation,it is recommended that tumor patients should be continuously infused with parenteral nutrition through the implantable venous port One time of flushing.Patients with lung cancer and gastric cancer have an increased risk of catheter blockage during PN infusion by cvpas.It is recommended that patients with lung cancer and gastric cancer should increase the frequency of catheter flushing during continuous parenteral nutrition infusion through implantable intravenous infusion port. |
