Objective: To observe the clinical application,matters need attention,complications and prevent measures of ultrasound-guided internal jugular vein implantation of access port.Methods: We collected 329 breast cancer patients who need for the adjuvant chemotherapy in The Second Affiliated Hospital of Dalian Medical University from2014.01 to 2016.12,Implantation of access port can be operation under local anesthesia.All the patients are female,age range from 28 to 72,average age is 47.8.All the patients finished examination before operation,no clotting disorders,no vein thrombosis,no sign of affection and irritability.The comparison of surgical complications in the subclavicular vein,femoral vein and Peripherally Inserted Central Catheter(PICC)in other parts.Results: 329 breast cancer patients finished the implantation of access port successfully,231 patiens are under ultrasound-guided,first puncture success was 95.2percent.Twice or more puncture success was 4.8%.The incidence of early complication was 1.2%.Long-term complication rate was 3.4%.The overall complication rate was 4.6%;98 of them have blind sight surgery,under one puncture success was 57.1%,twice or more puncture success was 42.9%.Compared with the femoral vein,the ultrasound-guided puncture of the internal jugular vein,the ?2 was8.52,P<0.05.Compared with the subclavian vein puncture group,?2= 23.52,P<0.05;Compared with the control group of PICC,?2= 25.12,P<0.05,the differencewas statistically significant.Conclusion: Ultrasound-guided internal jugular vein implantation of implantable access port,is a safe and effective,and reducing the pain of patients in the infusion method.
