| Objective To understand the status quo of medication compliance and quality of life in patients with vitiligo,to study the related factors affecting medication compliance,and to explore the differences in medication compliance and quality of life in patients with different TCM syndromes.Methods Patients with clinical diagnosis of vitiligo were selected to record their basic conditions and classify TCM clinical syndromes according to the 2018 Vitiligo Diagnosis and Treatment Consensus and vitiligo Treatment Expert Consensus and other relevant data.According to the Morisky Drug Compliance Scale(MMAS-8)and the Chinese vitiligo Specific Quality of Life Scale,the data of 189 vitiligo patients were collected by telephone return interview.Finally,SPSS21.0 software was used for logistic regression analysis,variance analysis,spearman correlation analysis and other statistical processing of the collected data.Results1.The average score of medication compliance in patients with vitiligo was(4.97±2.26),which was at a low level.110 cases(58.20%)had poor compliance,43 cases(22.75%)had moderate compliance,and 36 cases(19.05%)had high compliance.2.There were significant differences in medication compliance among patients with various TCM syndromes of vitiligo(P < 0.05).Among them,the medication literacy of patients with spleen and stomach weakness syndrome was significantly higher than that of other syndrome types,with an average score of 5.44±2.16,and that of patients with liver stagnation and qi stagnation was the worst,with an average score of 4.59±2.31.3.Logistic regression analysis showed that age,culture,stage,site and course of disease were independent risk factors affecting medication compliance(P < 0.05).The compliance of older patients was better than that of younger ones.The less educated are better than the more educated;Progressive stage is better than stable stage;The exposed site was better than the non-exposed site and the short course of disease was better than the long course of disease.There were no significant differences in the above independent risk factors among patients with vitiligo of different TCM syndromes(P > 0.05).4.The overall quality of life of patients with vitiligo is impaired.Patients often have negative emotional experience caused by leuplakia and worry about the spread of leuplakia,which significantly interferes with the quality of life.5.Patients with vitiligo of different TCM syndromes had different quality of life,and patients with liver stagnation and qi stagnation had the highest quality of life related scales,and their quality of life was affected the most.Among them,there were significant differences in quality of life between liver stagnation and qi stagnation syndrome,qi and blood disharmony syndrome and meridian block syndrome(P < 0.05).6.There was a moderate correlation between quality of life and compliance in patients with vitiligo.That is,the higher the quality of life index score is,the worse the patient’s medication compliance is;on the contrary,the patients with better quality of life also have better compliance.7.Patients with liver stagnation and qi stagnation vitiligo had the worst medication compliance and quality of life compared with patients with other syndroms.Conclusion:The medication compliance of patients with vitiligo needs to be improved.Clinical measures can be taken according to the above relevant influencing factors to improve the compliance of patients’ medical orders and achieve better color effect and quality of life.In addition,the medication compliance and quality of life of patients with liver stagnation and qi stagnation vitiligo are worse than those of patients with other syndromes.Doctors and patients’ families should provide appropriate scientific guidance and psychological comfort to patients with this syndrome. |
