Objective:This study investigated the factors affecting compliance with rivaroxaban in discharged patients with lower extremity deep venous thrombosis(LEDVT)and their association with quality of life.Main objective:1.To describe the current status and influencing factors of rivaroxaban compliance in discharged patients.2.To analyze the current status of medication beliefs and quality of life of discharged patients with LEDVT.3.To explore the correlation between medication compliance and quality of life in discharged patients with LEDVT.Methods:1.A convenient sampling method was used to select 179 discharged patients with LEDVT from vascular surgery in a third-grade hospital in Nanchang from March 2018 to November 2018.2.Using the general data questionnaire,medication compliance scale,medication beliefscale and quality of life scale to investigate the discharged patients with LEDVT,describe the patient's medication compliance status,analyze the factors affecting medication compliance and the relevance between medication compliance and quality of life.3.Statistical analysis was performed using SPSS17.0 software.The measurement data was expressed by x±s,and the count data was used.Cases and percentages(%)were described;univariate analysis was performed using chi-square test,multivariate analysis was performed using ordinal logistic regression,and correlation analysis was performed using the spearman correlation analysis.Results:1.Discharged Patients with LEDVThad low compliance(15.6%),moderate compliance(38%),higher compliance(46.4%),and medication compliance scores(7.13±1.00).2.Single factor analysis of drug compliance in discharged patients showed that different ages,working status,education level,medical insurance,monthly per capita income,home address,convenience in buying medicine,different incentives,side effects(bleeding),and combined chronic disease types were influencing factors in medication adherence(P(27)0.05).3.The results of ordinal logistic regression analysis showed that the main factors affecting the compliance of patients in discharge were age,work status,convenience in buying medicine,and medication beliefs.Compared with age-old(360 years old)patients,age(45~)years old patients were more inclined to take medication compliance;patients with jobs have higher medication compliance than those without job;Patients who are convenientto buy medicine have higher medication compliance;patients with a total medicationbeliefs score of more than 0 have higher medication compliance than those with a total score of less than 0.4.Follow-up results showed that 19 patients(11%)had not received rivaroxaban for 3 months,and 10,6 and 3 patients gave up rivaroxaban within 1,2,and 3 months after discharge.The main reason for patients who are discharged from lower extremity DVT to give up medication is that the drug price is more expensive.5.The average score of medication belief was(3.61±4.93),the average score of necessary belief was(16.82±2.92),the average score of concern belief was(13.21±3.06),and the average score of total quality of life of discharged patients was(50.51±3.77).The average symptom score was(50.39±4.70);the average score of physiological function was(49.14±2.64);the average score of physiological function(50.37±4.19);the average score of mental health was(48.64±3.36).6.Spearman correlation analysis was performed between quality of life and medication compliance in patients with LEDVT.The results showed that total quality of life,symptoms,physiological functions,physiological functions,mental health and medication were positively correlated(r=0.669,0.670,0.360,0.553,0.416,P<0.01).Conclusions:1.In this study,the adherence of medication was generally at a moderate level,with 15.6% of patients taking medication adherence at a low level.19 patients(11%)gave up rivaroxaban within 3 months after discharge,The patient's medication compliance needs to be improved.Follow-up missions to patients,emphasizing the importance of taking the drug,telling patients to take rivaroxaban without routine monitoring of coagulation indicators to save monitoring costs and return shipping costs;and the government is recommended to include rivaroxaban in the scope of medical insurance reimbursement.2.Age,work status,education level,medical insurance,monthly per capita income,home address,convenience in buying medicine,incentives,side effects(bleeding),combined chronic disease types,medication beliefs were the influencing factors of patient medication compliance.Among them,age(60 ~),no job,inonvenientto buymedicine,medication beliefs were the main influencing factor of low compliance of patients discharged from hospital,so medical staff should take medication compliance for the main factors affecting patient compliance.Management intervention.3.Patients with lower extremity DVT discharge are generally considered to have a belief in taking rivaroxaban.The quality of life of patients with lower extremity DVT discharge was moderate.4.There is a correlation between quality of life and medication adherence.The higher the patient's medication compliance,the higher the quality of life,and the quality of life can reflect the patient's disease recovery.
