Objectives To evaluate the predictive value of LDL-C cumulative exposure for recurrence of ischemic stroke,and to prevent recurrence of stroke.Methods A total of 637 patients with acute ischemic stroke with onset within 72 hours from September 2018 to September 2019 were selected.The subjects were all from the Department of Neurology,North China University of Technology Affiliated Hospital,and used this as an observation cohort.This study adopted a nested case-control study,followed up for 1 year,and divided into recurrence group(118 cases)and non-recurrence group(519 cases)according to the follow-up results.Through the collection of general information and laboratory tests and examination results of patients upon admission,NIHSS scoring were performed at the same time.The independent sample t test was used for measurement data,and the test was used for comparison of count data.Multivariate Logistic regression was used to analyze the correlation between the cumulative exposure level of LDL-C and the recurrence of ischemic stroke.Comparing the cumulative exposure of LDL-C and the application value of LDL-C by drawing the ROC curve of LDL-C cumulative exposure and Z test.And through paired chi-square test and consistency test,further evaluate the predictive effect of LDL-C cumulative exposure on stroke recurrence.Results 1 There was a statistically significant difference in the cumulative exposure of LDL-C between the relapsed group and the non-relapsed group(P<0.05).2 According to the standard of LDL-C cumulative exposure,it was divided into high and low level groups and the chi-square test was performed with the presence or absence of stroke recurrence.The results showed that there was a significant difference in the composition ratio of LDL-C cumulative exposure between the two groups(P<0.05).3 Multivariate logistic regression analysis showed that cumulative exposure of LDL-C,LDL-C and recurrence of ischemic stroke were significantly related.4 Analysis of the predictive value of LDL-C cumulative exposure to the recurrence of patients with acute ischemic stroke:The area under the ROC curve of LDL-C cumulative exposure was 0.952,the area under the LDL-C curve was 0.904,Z value was 4.156,P<0.001,the difference was statistically significant.The maximum Youden index of LDL-C cumulative exposure yields a diagnostic cut-off value of 3.5,a sensitivity of 0.923,a specificity of 0.967,an overall agreement of 0.959,a Kappa value of 0.868,and a paired chi-square test P=0.169(P> 0.05).5 The actual application effect of LDL-C cumulative exposure to predict the recurrence of acute ischemic stroke patients: sensitivity is 0.892,specificity is 0.915,overall consistency is 0.909,Kappa value is 0.773,paired chi-square test P=0.093(P>0.05).Conclusions The cumulative exposure of LDL-C has predictive value for recurrence of acute ischemic stroke.When cumulative exposure of LDL-C is greater than 3.Figure 1;Table 12;Reference 120...
