Objectives With the widespread prevalence of type 2 diabetes mellitus(T2DM),the number of T2 DM patients suffering from non-alcoholic fatty liver disease(NAFLD)is gradually increasing.Moreover,the occurrence of NAFLD may aggravate the adverse consequences of T2 DM patients,resulting in worse glucolipid metabolism and higher risk of macrovascular and microvascular complications in T2 DM patients.Studies in recent years have proved that bone morphogenetic protein-9(BMP-9)has been shown to have a positive effect on regulating glucose metabolism,lipid metabolism and energy metabolism in the body.The aim of this study was to investigate the relationship between BMP-9 and T2 DM with NAFLD.Methods 60 patients with type 2 diabetes mellitus in the department of endocrinology were selected and divided into 30 cases of T2 DM group and 30 cases of T2DM+NAFLD group.30 cases of normal control population and 30 cases of NAFLD population were selected from the physical examination center in the same period.All cases were recruited by Hebei General Hospital from March 2019 to January 2020.All the subjects were required to conduct detailed medical history collection and measurement of human parameters,including name,age,gender,height,fasting weight,drinking history,duration of diabetes,past history,systolic blood pressure(SBP),diastolic blood pressure(DBP),etc.,and are calculated body mass index(BMI).All the subjects’ blood samples were used to determine alanine aminotransferase(ALT),aspartate aminotransferase(AST),total cholesterol(TC),triglyceride(TG),high density lipoprotein cholesterol(HDL-C),low density lipoprotein cholesterol(LDL-C),apolipoprotein A1(Apo A1),apolipoprotein B(Apo B),fasting blood glucose(FBG),fasting insulin(FINS)and serum BMP-9 level,and were calculated homeostasis model assessment of insulin resistance(HOMA-IR).Results 1 Compared with the T2 DM group,the levels of BMI,TC,Apo B,FINS and HOMA-IR were significantly higher in the T2DM+NAFLD group(P<0.05),while the level of serum BMP-9 was significantly lower in the T2DM+NAFLD group(P<0.05).Compared with the NAFLD group,the levels of SBP,LDL-C,FBG and HOMA-IR were significantly higher in the T2DM+NAFLD group(P<0.05),while the level of serum BMP-9 was significantly lower in the T2DM+NAFLD group(P<0.05).Compared with the NC group,the levels of SBP,DBP,FBG,FINS and HOMA-IR were significantly higher in the T2 DM group(P<0.05),while the levels of HDL-C and serum BMP-9 were significantly lower in the T2 DM group(P<0.05).Compared with the NC group,the levels of BMI,DBP,TG,Apo B,FINS and HOMA-IR were significantly higher in the NAFLD group(P<0.05),while the levels of HDL-C and serum BMP-9 were significantly lower in the NAFLD group(P<0.05).2 Correlation analysis between serum BMP-9 level and related metabolic factors showed that the level of serum BMP-9 was negatively correlated with BMI(r=-0.382,P=0.000),SBP(r=-0.370,P=0.000),DBP(r=-0.242,P=0.008),ALT(r=-0.250,P=0.006),TC(r=-0.236,P=0.009),TG(r=-0.338,P=0.000),LDL-C(r=-0.283,P=0.002),FBG(r=-0.334,P=0.000),FINS(r=-0.509,P=0.000),HOMA-IR(r=-0.514,P=0.000),while the level of serum BMP-9 was positively correlated with HDL-C(r=0.299,P=0.001).3 Multiple stepwise regression analysis showed that FINS(B=-1.673,P=0.007),LDL-C(B=-15.529,P=0.002),HDL-C(B=45.936,P=0.007)and BMI(B=-2.656,P=0.032)were independent influencing factors of serum BMP-9 level.4 Logistic regression analysis showed that BMP-9(OR=0.939,95%CI: 0.899-0.981,P=0.005)was a protective factor for T2 DM with NAFLD,while HOMA-IR(OR=1.913,95%CI: 1.020-3.588,P=0.043)was a risk factor for T2 DM with NAFLD.Conclusions 1 BMP-9 may affect glucose metabolism and lipid metabolism and is closely related to insulin resistance(IR).HOMA-IR is a risk factor for T2 DM with NAFLD.2 Compared with patients with T2 DM without NAFLD,serum BMP-9 level is significantly lower in patients with T2 DM with NAFLD,and BMP-9 is a protective factor for T2 DM with NAFLD.Figure 0;Table 7;Reference 178...
