| Objectives To investigate the status and influencing factors of mental resilience in patients with acute coronary syndrome after PCI;A structural equation model of the relationship between the influencing factors of mental resilience was established,Analyze the mediating variables that influence mental resilience and the pathway relationship of interaction among the influencing factors,To provide the basis for nursing staff to formulate targeted intervention. Methods In the period from December 2019 to December 2020,Using the convenience sampling method,A total of 309 patients with acute coronary syndrome(ACS)were selected from a Grade A hospital in Tangshan,All the above patients underwent PCI.The contents of the questionnaire include:mental resilience scale, general data, disease data, social support rating scale, general self-efficacy scale,self-perceived burden scale, medical coping questionnaire,self-rating anxiety scale,self-rating depression scale, etc. The statistical software used for data analysis was SPSS17.0,Counting data were described by case number and percentage,Measurement data were described as mean±standard deviation.T-test and analysis of variance were used for comparison between groups,Pearson correlation analysis was used to analyze the relationship between mental resilience and various factors(such as social support, general self-efficacy,self-perceived burden),Multivariate factors were analyzed by linear regression,AMOS 24.0 software was used to construct the structural equation model. Results 1 Mental resilience of patients with acute coronary syndrome after,The score of mental resilience was(56.17±10.86),and the scores of various dimensions were as follows:perseverance(28.51±6.28),self-improvement(18.31±3.80)and optimism(9.35±1.69).2 Univariate analysis of mental resilience in patients with acute coronary syndrome after PCI,The results showed that gender, residence, occupation, education level, expense category, alcohol consumption, knowledge of disease, number of stent implantation, coping style, depression and other factors were related to mental resilience score,The difference was statistically significant(P<0.05).3 Pearson correlation analysis between social support and mental resilience,The results showed that there was a positive correlation between social support, general self-efficacy and mental resilience.Self-perceived burden of patients was negatively correlated with mental resilience.4Multivariate analysis of psychological resilience in patients with acute coronary syndrome after PCI:Multiple stepwise regression analysis showed that Social support, general self-efficacy,educational level and economic burden are the influencing factors of mental resilience in patients with acute coronary syndrome after PCI(R~2=0.604,P<0.05).5 Structural equation model path analysis results show:The model fit is good,NC(χ~2/df)=1.673,GFI=0.986,AGFI=0.957,NFI=0.984,IFI=0.994,CFI=0.994,RESEA=0.047.In the model,The total effects of general self-efficacy,social support, educational level and economic burden on mental resilience were 0.631,0.397,0.315,and-0.260.Education level and social support have direct and indirect positive effects on mental resilience.General self-efficacy has a direct positive effect on mental resilience.Economic burden has direct and indirect negative effects on mental resilience. Conclusions 1 Low level of mental resilience in patients with acute coronary syndrome after PCI.2 Patients with high educational level, social support level, high self-efficacy and low self-perceived burden had better mental resilience.3 The model shows that social support, general self-efficacy and self-perceived burden can directly affect patients’mental resilience, while coping can indirectly affect patients’mental resilience through social support and self-efficacy.Social support and self-efficacy were important mediating variables.Medical staff should pay attention to improving patients’personal qualities and combine with external forces such as family to improve patients’mental resilience and promote their physical and mental health.Figure 6;Table 21;Reference 150... |
