ObjectiveThe Qiqing Assessment Scale(QAS)and Life Event Scale(LES)were used to explore the correlation between TCM sentiment and thyroid nodule incidence,and the scales were used to quantitatively analyze the risk factors affecting thyroid nodule incidence,in order to guide the clinical prevention and treatment of thyroid nodule.MethodsThe clinical data of 128 patients with thyroid nodule and 128 normal healthy subjects who met the inclusion criteria in the Second Surgery Department of the First Affiliated Hospital of Guangzhou University of Chinese Medicine from June 2020 to February 2021 were retrospectively analyzed.The thyroid nodule group and the normal control group were matched according to the matching principle of similar age,body mass index(BMI),marital status,educational level,occupation category and personal living habits.A total of 128 patients were successfully matched.All the above 258 patients filled in the same basic information collection form,Qiqing Assessment Scale(QAS)and Life Event Scale(LES).The negative life events encountered by the patients in the past 5 years before the onset of the disease,the emotional changes experienced in dealing with the negative life events,the impact on the patients’ daily life and other aspects were recorded and quantified.The emotional factors affecting the incidence of thyroid nodule were classified and assigned,and the statistical relationship between the emotional factors and thyroid nodule was clarified,and the correlation between the high risk factors and the incidence of thyroid nodule was further calculated.Results1.There was no significant statistical difference in general conditions between the two groups(P>0.05),suggesting balanced baseline and comparability between groups.2.In the overall score of emotional factors of the Qiqing Assessment Scale between the two groups,the total score of happiness factor,anger factor,worry factor and thinking factor was significantly different between the two groups(P < 0.01),and the anger and thought factor of female patients with thyroid nodules was significantly different from that of male patients(P < 0.01).The regression analysis of negative temperament factors showed that anger factor(B=0.785,P<0.01,OR=2.192,95%CI :1.784--2.694),worry factor(B=0.282,P<0.01,OR=1.325,95%CI: 1.145--1.534),thought factor(B=0.276,P<0.01,OR=1.318,95%CI: 1.113--1.561).3.Among the emotional factors with positive detection rate of seven emotions higher than 95%,there were four factors of happiness,anger,worry and thinking in thyroid nodule group,while only two factors of happiness and thought in normal control group.The positive rate of seven emotions was divided into mild,moderate and severe groups.The nodular group was worry(68.22%),anger(45.74%),happiness,anger and thought(1.55%).The normal group were thought(72.09%),happiness(62.79%)and happiness(2.32%).4.Two groups of patients with negative life events content distribution has obvious statistical significance difference(χ ~2 =6.218,P<0.05).Negative life events stimulating strength results showed that the thyroid nodule group was significantly higher than that of normal control group(t = 6.845,P < 0.05).Conclusion1.In the five years before diagnosis,thyroid nodule patients were more affected by anger,worry and thought factors than the general healthy people,and in the multivariate analysis.Besides,the risk of anger,worry and thought factor was higher,and the main risk factor was anger.2.Among thyroid nodule patients,the factors and intensity of negative temperament stimulation are different in different genders,among which female patients are more likely to be affected by anger and thought factor than male patients.3.Thyroid nodule patients with positive emotional factors detected in happiness,anger,worry,thought,while generally healthy people detected in happiness,thought.And thyroid nodule patients with anger,anxiety,thought detected a higher degree of symptoms.4.Thyroid nodule patients suffer from more negative life events which were mainly marital and family problems,than normal healthy people.5.The stimulation intensity of negative life events in patients with thyroid nodule was much higher than that in normal people.
