| ObjectiveTo compare and analyze the differences of immunological function indexes among different TCM syndromes of patients with sepsis,and to explore the correlation between modern microscopic indexes such as m HLADR and lymphocyte count and TCM syndromes,so as to provide basis for early identification of immunosuppression in patients with sepsis.MethodsFrom June 2019 to January 2021,the patients with sepsis who were treated in the Department of Critical Care Medicine,the First Affiliated Hospital of Guangzhou University of Traditional Chinese Medicine,were eligible for inclusion in 54 patients.According to the dialectical of TCM,it was divided into heat toxin syndrome,blood stasis syndrome,acute deficiency syndrome and stomach excess syndrome.Immunological function indexes such as m HLA-DR,lymphocyte count,CD3+,T cell count,CD3+CD4+,Th Cells count,CD3+CD8+,TC Cells count,Th/Ts,CD3/16+56,NK Cells count,CD3/CD19,B Cells count,CD64 index and CD14+ were collected.Lymphocyte count and m HLADR were the main outcome indexes.The patients with sepsis were draw blood to test on the 1th,3th,and 7th day of admission.SAS 9.3 was used for statistical analysis of the data to observe the correlation between TCM syndromes and immunological function indexes.Results1.Fifty-four patients with sepsis were included,of which toxic heat syndrome accounted for 42.6%(23/54),blood stasis syndrome 5.6%(3/54),acute deficiency syndrome37.0%(20/54),and stomach excess syndrome 14.8%(8/54).2.The number of lymphocyte count in patients with sepsis with blood stasis syndrome was higher than that of other syndromes on the 1th and 3th day of admission.(P<0.01)The increase of lymphocyte count from the 3th to 7th day of admission in patients with sepsis with blood stasis syndrome was higher than that of other syndromes.(P<0.05)The increase of lymphocyte count from the 3th to 7th day of admission in patients with sepsis with acute deficiency syndrome was higher than that of stomach excess syndrome.(P<0.05)3.The number of T cell count in patients with sepsis with blood stasis syndrome was higher than that of other syndromes on the 3th day of admission.(P<0.01)4.The increase of Th Cells count from the 3th to 7th day of admission in patients with sepsis with stomach excess syndrome was higher than that of other syndromes.(P<0.05)5.There was no statistically significant difference in TCM syndromes and other immunological function indexes.Conclusion1.Among the patients with sepsis,the patients with toxic heat syndrome and acute deficiency syndrome were more,while the patients with blood stasis syndrome and stomach excess syndrome were less after TCM syndrome differentiation on the 1th day of admission.2.There was no statistically significant difference in m HLA-DR levels detected on the1 th day of admission among TCM syndromes.3.From the perspective of lymphocyte count detected on the 1th day of admission,the indexes of patients with blood stasis syndrome was not significantly decreased,while the indexes of patients with other syndromes was decreased in different degrees.From the actual data,compared with patients with other TCM syndromes,the number of lymphocyte count detected on the 1th day of admission of patients with blood stasis syndrome was higher.However,because there were too few patients with blood stasis syndrome included in the study,the conditions for statistical analysis were not available,and more cases were needed for statistical analysis,so as to obtain the corresponding conclusion,while there was no statistical difference in lymphocyte count among other syndromes. |
