Based On Data Mining Technology Of Treating The Decreasing Ovarian Reserve And Premature Ovarian Failure Treated By Professor Yuzhen Zhang Retrospective Study

PurposeDatabase of TCM inheritance platform and digital data mining technology are used in this research to analyze and conclude the trend of Professor Zhang Yu Zhen in treating ovarian dysfunction and premature ovarian failure,in order to inherit physician’s valuable clinical experience,and benefits patients by promote the development of clinical treatment.Ultimately,to promote the harmonious development of society.MethodsA total of 200 DOR and POF medical records and 743 prescriptions from July2012 to January 2018 in gynecology clinic were collected for retrospective study.Data collection is performed from medical records,information extraction and prescription medication.Data collation and double verification is executed,then data entry into the system of TCM supporting inheritance platform(V2.5).The modules of the software were used,which are "Statistical Report-Prescription Statistics","Data Analysis-Prescription Analysis",and "Data Analysis – Medical Case Analysis" to perform data mining analysis on the four major syndrome data and the overall data.Then,combined with Professor Zhang’s experience and clinical published academic experience to summed up the diseases diagnosis and treatment trend of Professor Zhang.Results1.The basic situation: 151 cases of POF accounted,49 cases of DOR accounted;patients average age is 27.78 years,minimum 14 years old,maximum39 years;mostly are 30-34 years old(35%),followed by 35-39 years old(33.5%).2.Epidemic situation: 17 cases of ovarian surgery history,11 cases of uterine fibroids,5 cases of recurrent miscarriage history,4 cases of rheumatoid arthritis,3 cases of mumps and systemic lupus erythematosus,2cases of chocolate cyst of ovary history,1 case of breast cancer after surgery.3.Types of syndromes: 743 prescriptions were divided into 10 syndrome types,the first four large syndromes accounted for a total proportion of93.81%: "Gan Sheng Yin Xu Xue Yu" frequency of 551 times,accounted for a proportion of 74.16%;"Qi Zhi Xue Yu" frequency of 63 times,accounted for a proportion of 8.48%;"Gan Sheng Yin Xu Huo Wang" frequency of 50 times,accounted for a proportion of 6.73%;"Pi Sheng Yang Xu Xue Yu" frequency of33 times,accounted for a proportion of 4.44%.4.Big Syndrome Data Mining4.1 Symptoms "Gan Sheng Yin Xu Xue Yu" 5 of the most frequent symptoms are vaginal discharge lesser,vaginal dryness,menstrual quantity lesser,menstruation postponed,Backache;10 of the most frequent used TCM are Dodder,Angelica,Salvia,Ligustrum,Codonopsis,Polygonatum,Morinda,Bupleurum,Rehmannia,White peony root.Drugs 4 Qi sorts are the warm,flat,cold,cool;drugs 5 flavors sort are Gan(sweet),Xin(spicy),bitter,sour;3 of the most frequent drugs "Gui Jing"(go through organs)are the liver,kidney and spleen.Set Support Level at the 5.44%,Confidence Level at 0.9,Result outcome of the most common TCM combinations are: Rehmannia,Dodder;Dodder,Salvia,Angelica;Dodder,Bupleurum,Cyperus rotundus and so on.4.2 Symptoms "Qi Zhi Xue Yu" 5 of the most frequent symptoms are breast tenderness,abdominal pain,backache,menstruation expired,menstruation postponed;10 of the most frequent used TCM are Peach kernel,Safflower,Angelica,Chuanxiong,Cyperus,Paeonia,Rehmannia,Millettia,Moutan,Pueraria;Drugs 4 Qi sorts are warm,cold,flat,cool;drugs 5 flavors sort are bitter,Xin(spicy),Gan(sweet),sour;3 of the most frequent drugs "Gui Jing"(go through organs)are liver,heart and spleen.Set Support Level of 15.89%,Confidence Level of 0.9,Result outcome of the most common TCM combinations are: Peach kernel,Safflower;Angelica,Chuanxiong;Angelica,Cyperus rotundus,Dodder and so on.4.3 Symptoms "Gan Sheng Yin Xu Huo Wang" 5 of the most frequent symptoms are "Zao Re Han Chu",insomnia,vaginal dryness,vaginal discharge less,menstrual postponed;10 of the most frequent used TCM are Dodder,Lily,Dogwood,Salvia,Rehmannia,Codonopsis,Ligustrum lucidum,White peony root,Radix Ophiopogonis,Ophiopogon japonicus,and Poria cocos;Drugs 4 Qi sorts are cold,flat,warm and cold;drugs 5 flavors sort are Gan(sweet),bitter,sour,Xin(spicy),Se(astringent)and salty;Set the Support Level of 10%,Confidence Level of 0.9,Result outcome of the most common TCM combinations are: Lily,Rehmannia;Lily,Anemarrhena;Cornus,Nacre and so on.4.4 Symptoms "Pi Sheng Yang Xu Xue Yu" 5 of the most frequent symptoms are menstrual quantity less,backache,menstrual postpone,lips pale and dark,mental and physical fatigue;10 of the most frequent used TCM are Angelica,Atractylodes,Dodder,Poria,Rehmannia,Codonopsis,Radix,Chuanxiong,Morinda officinalis,Eucommia;Drugs 4 Qi sorts are warm,flat,cold,cold;drugs 5 flavors sort are Gan(sweet),Xin(spicy),bitter,sour,Se(astringent),salty;Set Support Level of the9.09%,Confidence Level of 0.9,Result outcome of the most common TCM combinations are: Atractylodes,Poria;Codonopsis,Atractylodes,Morinda;Dodder,Morinda,Epimedium.5.New Drug Analysis Based on Entropy Clustering of Total Drugs Set the relevance of 8,the penalty of 5,draw a new drug combination of four:Millettia＿Peach kernel＿Codonopsis＿Moutan＿Red peony root,Poria＿Cornel＿Angelica＿Ophiopogon japonicus＿Lily,Ligustrum lucidum＿Salvia＿Polygonatum＿Red peony root,Cornus officinalis＿Angelica＿Ophiopogon＿Nacre＿Lily.Conclusion1.The two prescription drugs given by Professor Zhang Yuzhen in the treatment of DOR and POF,have no significant differences,according to the TCM theory "Bian Zheng Lun Zhi".The 743 prescriptions collected in this study is divided into 10 kinds of syndromes.2.Professor Zhang believes the main pathogenesis of the disease is "Gan Sheng Yin Xu Xue Yu",treat by nourish the liver,kidney,spleen,Qi,and Xue.When the patient has a sense of menstruation or have been consuming the prescription for a period of time,Professor Zhang likely to give "Li Qi Huo Xue" medicines for promote ovaries “Qi Xue”.When patients feel "Gan Sheng Yin Xu Huo Wang","Zao Re Han Chu","Li Qi Huo Xue",irritability,and insomnia,Professor Zhang will give "Zi Yin Qing Re" medicines.If symptoms such as patients having pale lips,afraid of cold,and limbs coldness,then "Pi Sheng Wen Yang" and "Yang Xue Huo Xue" medicines will be given.3.Professor Zhang’s clinical prescription mostly consist 12 flavors of TCM,that purpose to use lesser types of TCM to specialize the drug effects,the herbs combination is rigorous and "Zhu Ci Fen Ming".For example,the prescription used to nourish kidney,the TCM combination and dosage is depend on identify of "Qi Xue Yin Yang" and the degree of kidney wasting;the strong prescription used to nourish liver and kidney,as well as spleen and kidney,also including "Shu Gan Huo Xue",the TCM combination uses both Warm and Cold herbs to neutralize the side effects of the herbs.
