Objective:To investigate the differences in clinical features and recent outcomes of necrotizing enterocolitis(NEC)in premature infants,and to identify the characteristics and risk factors of operation between conservative and surgical treatment.Methods: A total of 73 children with NEC from November 2017 to November 2019 were collected for retrospective study,and they were divided into conservative group(n=40)and surgical group(n=33)according to treatment plan.The general data,laboratory indicators,imaging features and short-term prognosis of the two groups were comparedResults: There were no significant differences in gender,gestational age,birth weight,delivery mode,Apgar score and sepsis between the two groups(P>0.05),but in the merger,premature rupture of membranes,ductus open the difference was statistically significant(χ~2 =5.246,P=0.02;χ~2 =4.809,P=0.028).In terms of laboratory indicators,there were statistically significant differences in platelet,CRP and lactic acid between the two groups(Z=4.511,P=0.000;Z = 4.064,P = 0.00;Z=-3.317,P=0.001),there was no statistically significant difference in WBC and PCT(P>0.05).On abdominal X-ray,the detection rate of gas-liquid level changes and more than two changes in the surgical group was significantly higher than that in the conservative group(P>0.05).Conclusions: NEC children with premature rupture of membranes and open arterial ducts are more likely to progress to surgical treatment;patients with lower platelets,elevated CRP,elevated lactic acid,intestinal obstruction on the abdominal X-ray in the acute phase of NEC,or two or more changes of premature infants are more likely to progress to surgical treatment,and elevated lactate is a clear risk factor for NEC surgery.The time for children in the surgery group to reach full enteral feeding and adequate feeding is longer,so the hospital stay is longer...
