Objective: Objective to investigate the change trend and clinical diagnostic value of urinary α1 microglobulin and urinary microalbumin in the early stage of preeclampsia in elderly women,so as to provide basis and reference for clinical diagnosis and treatment.Methods: A retrospective analysis of 100 elderly puerpera(aged > 35)with relatively complete obstetric follow-up and delivery data from December 2017 to November 2019 in Heping Hospital of North theater general hospital was conducted.According to the guideline for diagnosis and treatment of hypertensive disorder complicating pregnancy(2015 Edition),32 cases of preeclampsia were selected as preeclampsia group;30 cases of pregnant women with gestational hypertension were set as gestational hypertension group;During the same period,38 cases of pregnant women with normal pregnancy examination were set as normal pregnancy group.The changes of urinary α1 microglobulin and urinary microalbumin were analyzed retrospectively after 20 weeks and every 4 weeks until 37 weeks of pregnancy.The immune room of the laboratory of our hospital used the immune scattering turbidimetric method to determine the urinary α1 microglobulin and urinary microalbumin in the random middle urine of elderly puerpera.Spss23.0 software was used for data processing,ANOVA of repeated measurement data was used,F test was used for measurement data,and mean was used ± Standard deviation(sx ?)showed that P < 0.05,the difference was statistically significant.Results: Among the elderly parturients,the levels of urinary α1 microglobulin and urinary microalbumin in preeclampsia group were(13.94 0 ± 5.60、35.94 ± 24.23)mg/L、(20.67 ± 11.39、147.35 ± 142.52)mg/L、(31.11 ± 18.71、223.80 ± 218.16)mg/L、(44.95 ± 33.36、348.03 ± 341.78)mg/L、(61.86 ± 49.96、457.93 ± 453.51)mg/L、(65.49 ± 50.10、569.44 ± 98)mg / L,which were higher than those in the pregnancy induced hypertension group and normal pregnancy group(P < 0.05);In preeclampsia group,the levels of urinary α1 microglobulin and urinary microalbumin in severe preeclampsia patients at 20,24,28,32,34 and 37 weeks of gestation were(18.98 0 ±8.46、71.02 ± 39.61)mg/L、(31.12 ± 17.95、377.40 ± 205.55)mg/L、(52.82 ± 31.23、552.48 ± 362.53)mg/L、(86.02 ± 63.31、916.12 ± 482.28)mg/L、(135.55 ± 88.93、1228.00 ± 607.73)mg/L、(65.49 ± 50.10、1483.33 ± 84)mg / L,higher than that in patients with mild preeclampsia(P < 0.05).Conclusion: From 20 weeks of gestation,the levels of urinary α1 microglobulin and Microalbumin in preeclampsia patients were higher than those in hypertensive patients and normal pregnant women.It is suggested that from the 20 th week of gestation,the levels of urinary α1 microglobulin and urinary microalbumin in preeclampsia patients are increased,and they are significantly increased with the increase of gestational age.The degree of increase may reflect the severity of the disease.
