| Objective In this study,the clinical and imaging date of domestic 3D-ACT acetabular cup and traditional acetabular cup were retrospectively analyzed to evaluate the short-term clinical effect of domestic 3D-ACT trabecular cup in total hip arthroplasty,and provide ideas for the improvement and exploration of domestic acetabular prosthesis.Methods A total of 69 patients were collected and divided into two groups:domestic 3D-ACT group: domestic 3D-ACT acetabular cup;Pinnacle group: traditional Pinnacle acetabular cup.Domestic 3D-ACT group had 38 patients(38 hips),Pinnacle group had 31 patients(31 hips).Follow-ups were performed at before operation and postoperative 1week,3,6,12 months for two groups,including visual analogue score(VAS),harris hip score(HHS),forgotten joint score(FJS),operation time,intraoperative blood loss,postoperative hospital stay,postoperative time of discarding the crutches,postoperative complications,postoperative satisfaction,the stability of the acetabulum prosthesis and bone ingrowth.Results All patients received complete follow-up,and no cases were lost to follow-up or died.(1)Results of general follow-up: there was no significant difference in preoperative HHS and VAS score between the two groups(P > 0.05),there was significant difference in postoperative 1 week HHS and VAS score between the two groups(P<0.05),and there was no significant difference in postoperative 1,6 and 12 months HHS and VAS scores between the two groups(P>0.05).In postoperative 12 months,domestic 3D-ACT group HHS score from preoperative(35.08 ± 4.74)points up to(94.50 ± 3.21)points,of which more than 90 points in 35 cases,fine rate was 92.11%,and VAS score from preoperative(5.42 ± 1.11)points to 0 points,HHS score of Pinnacle group from preoperative(34.19 ± 4.48)points to(93.26 ± 2.92)points,28 cases in which more than 90 points,fine rate was 90.32%,and VAS score from preoperative(5.87 ± 1.28)points to 0 points,there was significant difference from preoperation to postoperation in HHS and VAS score between the two groups(P <0.05).There was no significant difference in postoperative 6 and 12 months FJS score between the two groups(P > 0.05).There was no significant difference in postoperative time of discarding the crutches and postoperative complications between the two groups(P>0.05).All the thrombus in postoperative complications were superficial vein thrombus,which were improved after treatment.The dislocation occurred in 1 patient on the postoperative 4th day and improved after treatment.At the final follow-up,the postoperative satisfaction of patients in the domestic 3D-ACT group was 81.58%,and the satisfaction of patients in the Pinnacle group was 83.87%,there was no significant difference between two the groups(P>0.05).And asked about the reasons for dissatisfaction,roughly divided into two categories: first,uncomfortable weather: when it rains or cold damp environment,patients will feel the acid swelling feeling at the joint replacement site;second,can not walk long distance: when persistently walking distance is more than 1 kilometer,the patients will feel uncomfortable in joint replacement site.(2)Perioperative evaluation: there was significant difference in postoperative length of hospital stay between the two groups(P < 0.05),there was no significant difference in operative time and intraoperative blood loss between the two groups(P>0.05).(3)Imaging evaluation: two groups in postoperative follow-up period,the hip X-ray of acetabular cupⅠ,Ⅱ,Ⅲ area was no radiological translucency lines,the position of the acetabular prosthesis was satisfactory,no obvious signs of shift were observed,and the fixation was stable,good bone growth was observed between the cup and the bone interface.Conclusion The use of domestic 3D-ACT cup for the first THA,the initial stability of the cup is higher,and the bone growth between the cup and the bone interface is good,which can enable patients to obtain good hip function,short-term clinical effect is satisfactory,but further follow-up is needed to evaluate the long-term effect. |
