Objective: Total hip arthroplasty is one of the most effective treatments for end-stage disease in the hip today.In hip replacement/ revision surgery,the reconstruction of the acetabular center of rotation and the initial stabilization of the acetabular implant are particularly important compared to a slightly "processing" procedure on the femoral side.In clinical work,acetabular reconstruction of congenital hip dysplasia and severe pelvic defect revision,as an existing recognized problem,always troubles joint surgeons.This study is based on the above clinical difficulties,to explore the value of 3D printed pelvic model in the reconstruction of severe acetabular defects.Methods: Collect patients from Shengjing Hospital affiliated to China Medical University from December 2012 to December 2018,Crowe III,IV DDH patients and patients undergoing revision surgery for severe bone defects.The experimental group imported the MIMICS software to create a 3D digital model based on the patient's hip spiral CT 1mm thin layer plain scan + 3D results in DICOM format file,save it in STL file format,and create a 3D print model of the patient's acetabulum before surgery.Plan,simulate surgical procedures.The control group underwent routine total hip arthroplasty/ revision surgery.The hip function of the two groups were compared with Harris score,Vas score,false external angle,the operation time and intraoperative blood loss of the two groups.Among them,DDH study has a total of 24 hips in 20 patients,including 3 males and 17 females,with an average age of 52.65 years old.A total of 11 patients(12 hips)were studied in the revision study,including 6 males and 5 females,the mean age is 62 years old.Results: In the two groups of patients with different studies,Comparing Harris score,Vas score,abduction angle,operation time,intraoperative blood loss,etc.,the experimental group was significantly better than the control group,the difference was statistically significant(P <0.05).Conclusion: The 3D printed acetabular model can be used in the severe replacement/renovation of acetabular defects.It can be preoperatively planned according to the acetabular 3D printing model,predicting the degree of bone defect and predicting the type of acetabular prosthesis,thus reducing the operation time and Intraoperative hemorrhage.The postoperative stability of the implant is better,hip function recovery is better.So 3D-printing pelvic has great application value in acetabular defect reconstruction of hip replacement/renovation.
