Purpose: 1.To summarize the incidence of cholestasis of premature infants in our hospital;2.To explore the influencing factors of cholestasis of premature infants.Method: All premature infants born from November 2018 to December31,2018 and admitted to NICU in the second Hospital of Hebei Medical University were selected as the observation group.The premature infants diagnosed with cholestasis during hospitalization were selected as the observation group,and the premature infants with the same number of non-cholestasis in the same period,gestational age(GA)and birth weight(BW)similar to those in the observation group were randomly selected as the control group(except those with incomplete data).Collect clinical data and laboratory data,compare and analyze them.Results:1.The incidence of cholestasis in premature infants was 3.73%(54/1449),The incidence was negatively correlated with gestational age and body weight,and there was no significant difference in the incidence of cholestasis in the past three years.2.Compared with the control group,the incidence of PDA and septicemia in the case group was higher than that in the control group(P=0.032 and P<0.001).Peripherally inserted central catheter(PICC)use time,hospital stay,fluconazole use time,parenteral nutrition(PN)duration and fasting time were longer than those in the control group,and the milk opening time was later than that in the control group.The accumulation of three substances(glucose,amino acid,fat emulsion),the average calorie intake at(TPN)and the total calorie intake at TPN were significantly higher than those in the control group.Septicemia and fasting time were predictive risk factors for cholestasis of preterm infants.The incidence of cholestasis in children with septicemia was 7.495 times higher than that in children without septicemia.OR for 7.495,95%CI(1.924,29.192),For every one day of extended fasting time,the incidence of cholestasis increased by 52.2%,OR for1.522,95%CI(1.129,2.052).3.The cause of cholestasis of preterm infants was complicated.Parenteral nutrition-associated cholestasis(PNAC)accounts for 51.85% of cholestasis of premature infants,which was the main cause of cholestasis of premature infants.Conclusion:1.The incidence of cholestasis in premature infants with NICU in our hospital was 3.73%.2.Septicemia and fasting time were predictive risk factors of cholestasis in premature infants.3.PNAC was the main cause of cholestasis of premature infants.
