Objective: To investigate the clinical efficacy of repetitive transcranial magnetic stimulation(r TMS)for the treatment of convulsive status epilepticus(CSE).Methods: Forty patients were enrolled and randomly divided into control group and r TMS group,including 20 cases in the control group and20 cases in the r TMS group.The patients were cured with diazepam in the control group;The patients in the r TMS group was given low-frequency r TMS on the basis of control group,1-2 times a day.The clinical efficacy in the two groups was evaluated after 48 hours of treatment,and the effective rate of each group was calculated.The STESS scores,plasma levels of neuropeptide Y(NPY)and Video EEG changes before and after treatment were compared between the two groups.Both groups were treated in epilepsy monitoring unit(EMU)and monitored by bedside VEEG.Results: 1 There were no significant differences in age,gender,and duration of disease between the two groups.2 The effective rate of the control group was 55%,and the effective rate of the r TMS group was 85%.The r TMS group was significantly higher than the control group,and the difference was statistically significant.3 The STESS scores of the two groups decreased significantly after treatment compared with before treatment(P<0.05).After treatment,the STESS score in the r TMS group was significantly lower than that in the control group.The difference was statistically significant(P<0.05).4 Before treatment,there was no significant difference in plasma NPY between the two groups(P>0.05).After treatment,the plasma NPY of the two groups were significantly higher than those before treatment(P<0.05)and plasma NPY in the r TMS group was significantly higher than in the control group(P<0.05).5Comparison of changes of video electroencephalogram before and after treatment between two groups,the effective rate of EEG improvement in r TMS group was 90%(18/20),higher than that in the control group50%(10/20),and the difference was statistically significant(P<0.05).Conclusion: Repetitive transcranial magnetic stimulation is safe and effective in the treatment of CSE.It is worthy of further clinical promotion.
