Background:With the continuous development of implant repair technology and the update of various new materials and technologies,single jaw implant fixed repair has become an ideal repair method for patients with dentition loss.The early implant field focuses on the implant survival rate,while the superstructure related process complications are often ignored.However,maintaining the long-term stability of dental implant prosthesis is closely related to the upper prosthesis materials.The upper part of the prosthesis material will affect the complications,masticatory efficiency,aesthetics,self-cleaning effect and long-term retention rate.With the continuous upgrading of superstructure materials,it is inevitable that there will be some process complications that are perplexing the majority of patients and doctors,which will reduce the satisfaction of patients with implant repair,and increase the clinical workload of doctors.In view of the complications of the upper structure of single jaw fixed implant denture,this paper analyzes and compares the design,material selection and manufacturing technology of different upper structures,and finds a kind of repair method with high masticatory function,good aesthetics,less complications and long-term stability,which is the current hot spot of single jaw edentulous implant repair.Objective:To analyze the process complications of different implant superstructure materials and designs in single jaw edentulous cases,and compare the clinical effects,characteristics and limitations of different implant superstructure materials and designs,so as to provide reference for the application of different implant superstructure materials and designs in single jaw fixed implant repair.Methods:The cases of missing single jaw dentition due to periodontitis and caries in stomatology department,the First Affiliated Hospital of Guangzhou Medical University from 2016 to 2019 were collected retrospectively.According to the preoperative imaging measurement and oral examination,a personalized implant repair plan was developed.6-7 strauman,Ankylos,ICX,Anyone implants were implanted axially.After 4-6 months,the implant fixed denture was restored by using CAD / CAM cutting pure titanium bracket external bonding single crown,CAD /CAM cutting pure titanium bracket resin composite fixed bridge,cobalt chromium casting bracket porcelain fused to metal crown bridge restoration.One year after permanent restoration,the follow-up was carried out to understand the incidence rate of prosthesis process complications(bracket fracture,artificial tooth collapse,artificial tooth fall off),the success rate of implant,and the clinical effect Implant modified plaque index,implant modified bleeding index and subjective satisfaction of patients were used to evaluate the application effect and limitations of different implant superstructure materials and designs in single jaw fixed implant repair.Among them,CAD / CAM cutting pure titanium bracket external bonding single crown repair was group A,CAD / CAM cutting pure titanium resin composite fixed bridge repair was group B,cobalt chromium casting bracket porcelain fused to metal crown and bridge was group C.ResultsIn this study,we collected 9 cases of single dentition missing due to periodontitis and caries,including 6 cases of maxillary implant supported fixed prosthesis and 3 cases of mandibular implant supported fixed prosthesis;A total of56 implants were implanted to repair 112 working teeth.There were 9 permanent restorations,including 3 in group A.19 implants were implanted to restore 40 working teeth.All of them were female patients with an average age of(54±2.1)years old;In group B,18 implants were implanted and 36 working teeth were restored.all male patients with an average age of(62±1.6)years old;In group C,36 working teeth were restored with 19 implants in 3 pairs,all male patients with an average age of(58±2.5)years old.1.Within 12 months after the permanent restoration,the fracture rate of implant fixed prosthesis in the three groups was 0%.2.Within 12 months after permanent restoration,the collapse rate of artificial teeth in group A was 10%,that in group B was 19.4%,that in group C was 33.3%,2×3 Chi square test showed that P=0.042,the difference was statistically significant(P < 0.05);Bonfreund’s method was used to compare between groups,and the difference between group A and group C was statistically significant(P < 0.05),while the difference between group A and group B,group B and group C was not statistically significant(P > 0.05).3.Within 12 months after permanent restoration,the rate of artificial teeth falling off was 5% in group A,8.3% in group B,0% in group C and 2×3 Chi square test showed that P = 0.32,the difference was not statistically significant(P > 0.05).4.Within 12 months after permanent restoration,the implant success rate of the three groups was 100%.5.Within 12 months after implant restoration,the modified plaque index of group A was 0.74±0.20;The modified plaque index of group B was 0.94±0.22;The modified plaque index of group C was 0.89±0.22.6.Within 12 months after implant restoration,the mean value of implant modified bleeding index in group A was 0.53±0.14;In group B,the mean value of implant modified bleeding index was 0.67±0.17;In group C,the mean value of implant modified bleeding index was 0.89±0.20.7.In terms of subjective satisfaction,masticatory satisfaction: group A > group C > group B(P < 0.05,with statistical significance),aesthetic satisfaction: group A >group B > group C(P < 0.05,with statistical significance),comfort satisfaction:group A > group B > group C(P < 0.05,with statistical significance).Conclusions:Compared with CAD / CAM cutting pure titanium stent resin composite fixed bridge and cobalt chromium casting stent porcelain fused to metal crown and bridge,CAD / CAM cutting pure titanium stent resin composite fixed bridge and cobalt chromium casting stent porcelain fused to metal crown and bridge have certain advantages,It is an effective and good upper structure design of single jaw implant fixation.However,the tooth loss rate is high,which needs further evaluation.
