Study On Wenyang Tongbi Decoction Combined With Moxibustion Separated From Aconite Cake In The Treatment Of Uric Acid Nephropathy Of Spleen Kidney Yang Deficiency And Cold Dampness Obstruction Type

Objective:There are three types of uric acid nephropathy:acute uric acid nephropathy,chronic uric acid nephropathy and uric acid stones,Clinically,chronic uric acid nephropathy is the most common disease,Fromthe clinical research,through observing the clinical effect and laboratoryindexes of the treatment of chronic urinary acid nephropathy with spleen kidney yang deficiency and cold wet obstruction by Wenyangtongbi recipe combined with Moxibustion with separated aconite cake,and to evaluate its safety,provide new diagnosis and treatment methods for the disease.Methods:60 patients with chronic uric acid nephropathy in our hospital and met the diagnosis criteria of spleen-kidney yang deficiency and cold dampness obstruction pattern were selected and divided into control group and treatment group by random number method,30 cases in each group.In the control group,40 mg febuxostat tablets were taken orally once a day for 8 weeks;The treatment group was treated with Wenyang Tongbi self-made prescription,one dose of decoction 300ml a day,warm clothes after breakfast and evening,combined with moxibustion thera py separated by aconite cake:bilateral Pishu,Shenshu and Mingmen acupoints were selected,the patients were asked to take prone position,fully expose the acupoints to be moxibustion,dipappropriate amount of ginger juice with cotton ball,rub the selected acupoints once,and place a single layer of gauze on five acupoints,Then the prepared aconite cake was put on,and a cone-shaped large moxa column with a bottom diameter of 2cm and a height of 2cm was prepared by using a customized grind ing tool.The moxa column was placed on the aconite cake,ignited,and moxibustion was carried out once every three days,three strong each time,about 10min each time.The treatment lasted for 8 weeks with ruddy skin and noblistering.According to the changes of TCM syndrome score before and after the experimental study,the curative effect indexes included serum uric acid(SUA),serum creatinine(SCR),blood urea nitrogen(BUN),serum cystatin C(Cys C)and urine β2-microglobulin(β2-MG),24-hour urinary protein(24UPQ)and pain degree(VAS score)were measured.Results:1.The treatment group was significantly better than the control group in improving TCM syndrome score,with statistical significan ce(P<0.05).2.SUA,SCR,BUN,Cys C,β2-MG,24 h UPQ and pain VAS score of the treatment group were lower than those of the control group,with statistical significance(P<0.05).3.The overall effective rate of TCM syndromes in the treatment group and the control group were 93.33% and 80% respectively,which showed the treatment group had better overall effective rate with statistical significance(P<0.05);The overall effective rate of Western Medicine in the treatment group and the control group were 93.33% and 80% respectively,which showed the treatment group had better overalleffective rate with statistical significance(P<0.05).4.No severe adverse or allergy-related events occurred in either group.Conclusion:1.Wenyang Tongbi Decoction Combined with Moxibustion Separated from aconite cake has significant clinical effect on chronic uric acid nephropathy of spleen kidney yang deficiency and cold dampness obstruction type.2.Wenyang Tongbi Decoction Combined with Moxibustion Separated aconite cake can improve the TCM syndrome score,SUA,Scr,BUN,Cys C,β2-MG,24 HUPQ and pain VAS score of patients with chronicuric acid nephropathy of spleen kidney yang deficiency and cold dampness obstruction type.
